Conditions de réalisation des études, détresse psychologique, intention d'abandonner le programme et stratégies de soutien durant la formation infirmière : une étude en deux phases auprès d'une population étudiante de deux milieux d'enseignement supérieur du Québec

Abstract
« La détresse psychologique et l'intention d'abandonner le programme sont répandues chez les personnes étudiantes du domaine infirmier. Les conditions de réalisation des études, soit les stresseurs psychosociaux et les difficultés de conciliation études-travail-vie personnelle, pourraient y contribuer. Cette recherche en deux phases auprès de personnes étudiantes du domaine infirmier (cégep et université), visait à : 1) Estimer la prévalence des conditions de réalisation des études, de détresse psychologique et d'intention d'abandonner le programme ; 2) Examiner les associations entre les conditions de réalisation des études et a. la détresse psychologique ; b. l'intention d'abandonner le programme 3) Explorer les stratégies de soutien pour améliorer les conditions de réalisation des études en réunissant les perspectives des personnes concernées. Les modèles théoriques de Karasek (1979) et de Siegrist (1996) encadrent cette recherche. Dans la phase quantitative, 230 personnes ont complété un questionnaire afin d'estimer les prévalences (objectif 1). Des modèles de Poisson robuste ont été construits pour examiner les associations entre les variables (objectif 2). Les efforts élevés, les difficultés de conciliation, le surinvestissement et la détresse psychologique étaient fréquents (49,5-84,7 %). Le surinvestissement et les difficultés de conciliation étaient associés à la détresse psychologique (rapport de prévalence ajusté (RPA) 2,10 ; intervalle de confiance à 95 % (IC) 1,15-3,84 et RPA 2,32 IC 1,24-4,32). Le déséquilibre efforts-reconnaissance et les difficultés de conciliation étaient associés à l'intention d'abandonner le programme (RPA 2,32 IC 1,09-4,94 et RPA 2,40 IC 1,05-5,45). Dans la phase qualitative (objectif 3), deux entretiens de groupe ont été menés dans chaque milieu ciblé auprès des personnes étudiantes (6) et de membres du personnel (10). L'analyse thématique de Paillé et Mucchielli (2016) a permis de faire émerger une catégorie de stratégies de soutien commune aux deux milieux, « favoriser la conciliation études-travail-vie personnelle », et deux autres catégories, soit « soutenir la poursuite aux études collégiales » et « faciliter la transition entre le DEC et le BAC ». Effort-reward imbalance and school-work-life conflicts were associated with intention to drop out of the program (RPA 2.32 CI 1.09-4.94 and RPA 2.40 CI 1.05-5.45). In the qualitative phase (objective 3), two group interviews were conducted in each targeted nursing school with students (6) and staff members (10). The thematic analysis by Paillé and Mucchielli (2016) allowed to identify a category of support strategies common to both settings, "promote school-work-life balance", and two other categories, namely "support the pursuit of college studies" and "facilitate the transition between the DEC and the BAC".
