Achtergrond: De insulineshocktherapie werd vanaf de jaren dertig van de 20e eeuw toegepast bij patiënten met een klinische diagnose schizofrenie. Hoewel deze therapie theoretisch en empirisch nauwelijks onderbouwd was, werd deze enthousiast ontvangen en veelvuldig toegepast, om in de jaren vijftig geruisloos uit het behandelrepertoire te verdwijnen.<br/><br/>Doel: Bieden van inzicht in de ervaringen met de insulineshocktherapie in de Haagse psychiatrische klinieken en in de factoren die bijdroegen aan de opkomst en ondergang ervan.<br/><br/>Methode: Literatuuronderzoek in PubMed, Embase, Medline en de historische literatuur met als zoektermen ‘insulin shock treatment’ en ‘insulin coma therapy’, alsmede onderzoek van medische dossiers en andere relevante documentatie uit de voormalige Haagse psychiatrische klinieken Rosenburg en Bloemendaal.<br/><br/>Resultaten: In Den Haag werd in 1937 gestart met de insulineshocktherapie. De verbeteringspercentages, gemeten volgens de richtlijnen van de Canadian Committee for Mental Hygiene, waren goed. Ernstige complicaties werden relatief weinig beschreven. Het gebrek aan insuline tijdens de Tweede Wereldoorlog en personeelstekorten daarna hadden een negatieve invloed op de uitvoering van de therapie.<br/><br/>Conclusie: De insulineshocktherapie in Den Haag werd net als elders met veel succes toegepast. Het verdwijnen van de therapie in Den Haag lijkt meer op praktische gronden te hebben berust dan op evidence-based overwegingen.<br/>
