Riassunto Parole chiave: otite esterna, cane, indagine clinico-epidemilogica. L’otite esterna è una comune malattia degli animali d’affezione, definita come l’infiammazione acuta o cronica del padiglione auricolare e del condotto uditivo esterno. Tutte le razze, di qualsiasi sesso e ad ogni età possono esserne soggette. Questa patologia spesso rappresenta un problema di difficile risoluzione per il veterinario ed un problema frustrante per il proprietario dell’animale. Il decorso della malattia spesso è lungo, le recidive sono frequenti e talvolta la terapia risulta inefficace, facendo progredire il caso verso un’otite esterna terminale. Ha un'eziologia multifattoriale; l’evento scatenante infatti può essere rappresentato da un singolo agente causale oppure può essere il risultato della somma di più fattori. Lo schema usato per la classificazione degli agenti scatenanti ha subito negl’anni diversi modificazioni, ma prende origine dallo schema proposto nel 1988 da JR August, che per primo riportò nella classificazione non solo le cause ma anche i fattori, distinti in predisponenti e perpetuanti. L’infiammazione del canale auricolare produce una cascata d’eventi che rendono l’orecchio un ambiente favorevole alla moltiplicazione dei micro-organismi patogeni. Lo stato infiammatorio cronico indotto da otiti ricorrenti può provocare gravi modificazioni del condotto uditivo e nei casi più gravi si può osservare l’ossificazione della porzione cartilaginea. Queste modificazioni sono frequenti cause d’interventi chirurgici della rimozione del condotto perché la terapia medica diviene inefficace. In questo studio sono stati valutati i casi clinici di cani affetti da patologie auricolari che si sono presentati all’ospedale “Mario Modenato” di S Piero a Grado dal 2010-2014. L’obbiettivo della seguente tesi è stato quello di analizzare in via retrospettiva e prospettiva i casi di otite esterna nel cane al fine di ottenere dati clinico epidemiologici riguardanti il segnalamento, le cause, fattori predisponenti e perpetuanti. Nello studio sono stati considerati l'età, la razza e il sesso; la causa primaria, gli eventuali fattori predisponenti e l'associazione tra i patogeni, cocchi, bastoncellari e malssezie. Dalla valutazione dei casi è risultata una prevalenza delle femmine (52,74%) sui maschi (47,26%). Le razze maggiormente predisposte allo sviluppo di otite nella nostra popolazione sono il Labrador (12,39%), il pastore tedesco (8,72%), il Bulldog inglese (6,42%), e il cocker spaniel (4,59%). I 219 cani che presentavano otite esterna avevano quale malattia predisponente: dermatite atopica (74,43%), parassiti (6.85%), corpi estranei (6,39%), endocrinopatie (4,57%), patologie del padiglione (2,74%), otite media (2,28%), enteriti croniche (1,37%), allergia al morso da pulce 0,91%), leishmania (0,46%). Otits externa is a common diagnosed disorder in the dog, generally known as the acute and chronic inflammation of the ear canal end pinna. All breeds, of both genders and of any age can be affected by it. This disorder is often a challenge for the veterinary to resolve and a source of frustration for the client. Long recurrent disease and failure of treatment often occurs, progressing eventually into an end-stage otitis. The underlying reasons for otitis are various, and the primary cause may be single or related to other factors. The classification scheme has been adapted and updated within the years, but it is inspired by the original scheme proposed by JR August in 1988, in which for the first time factors, predisposing and perpetuating, where introduced. The inflammation of the ear canal allows the development of an appropriate microclimate for the overgrowth of pathogenic microrganisms. Chronic inflammation status and recurrent otitis, may end in severe modification of the ear canal, and may lead up to ossification. Since medical treatment is not effective in these cases, such modifications become the main reason for surgery. This study was undertaken on canine patients form the veterinary hospital of “Mario Modenato” in S Piero a Grado, Pisa from 2010 until 2014. The aim of this study was to analyze cases of otits externa to obtain epidemiological results such as signalling, primary causes, predisposing factors and perpetuating factors. Age, sex, breed, primary causes, factors and association between the three pathogens cocci, rods and yeast were all taken in consideration. The largest breed distributions in this study are: Labrador retrievers (12,39%), German shepherd (8,72%), Bulldog (6,42%) and cocker spaniel (4,59%). The 219 dogs that had otitis were diagnosed with: atopic dermatitis (74,43%), parasites (6.85%), foreign bodies (6,39%), endocrinopathy (4,57%), disorders of the pinna (2,74%), otitis media (2,28%), chronic enteritis (1,37%), allergy from flee bites (0,91%), leishmania (0,46%).
