Abstract
Les maladies cardiovasculaires demeurent une des principales causes de mortalité, bien que les recherches des dernières décennies aient permis d'améliorer considérablement les techniques d'interventions chirurgicales et pharmacologiques.Parmi les nombreux facteurs impliqués dans la dysfonction du coeur et la progression de la maladie, on retrouve les perturbations du métabolisme énergétique.À cet égard, cette étude vise à caractériser le processus de cataplérose du citrate, un phénomène défini comme étant le transfert du citrate de la mitochondrie au cytosol, et documenter sa régulation dans le coeur intact sain et malade.L'intérêt pour le citrate, un métabolite mitochondrial crucial à la production d'énergie par le cycle de Krebs, a été suscité par des études démontrant que le myocarde de patients atteints de maladies coronariennes libère une quantité de citrate proportionnelle à la sévérité de la maladie.En outre, il est également proposé que le citrate cytosolique joue un rôle clé dans la sélection des substrats énergétiques utilisés par le coeur.L'approche expérimentale utilisée est le coeur de rat perfusé ex vivo en présence de substrats énergétiques marqués au carbone 13 et l'analyse des isotopomères de masse par chromatographie gazeuse-spectrométrie de masse.Les mesures d'efflux de citrate ont été corrélées avec des paramètres fonctionnels et métaboliques telle que la contribution relative des substrats énergétiques à la synthèse du citrate.Les résultats de cette thèse démontrent que la libération de citrate par le coeur perfusé ex vivo -de l'ordre de 5 à 21 nmol.g1.min1 et représentant < 2% du flux du cycle de Krebs reflète le transfert du citrate de la mitochondrie au cytosol.Ce processus (i) est spécifique, (ii) n'est pas corrélé avec un indice de nécrose, (iii) peut être modulé par la demande énergétique et l'apport en substrats exogènes pour la synthèse de citrate, (iv) est augmenté chez les coeurs hypertrophiques, (y) semble atteindre un niveau maximal en condition physiologique, possiblement limité par la faible activité du transporteur mitochondrial et/ou plasmatique.Les résultats de cette thèse renforcent l'importance de clarifier le rôle du citrate cytosolique comme molécule signal, spécifiquement comme précurseur du malonyl-CoA, un régulateur de l'oxydation des acides gras d'un grand intérêt clinique.Mots clés : métabolisme énergétique, cycle de Krebs, perfusion ex vivo de coeur de rat, chromatographie gazeuse couplée à la spectrométrie de masse, hypertension et hypertrophie SUMMARY Cardiovascular diseases remain one cf the principal causes cf hospitalization, morbidity and mortality, although researches over the past decades have considerably improved the surgical techniques and the pharmacological interventions.Alterations in energy metabolism have been found te be implicated in heart dysfunction and disease progression.Thus, the aim cf the present study was te characterize the process cf citrate cataplerosis, a phenomenon defined as the citrate efflux from the mitochondria to the cytosol, and to document its regulation in the intact heart.This interest for citrate, a mitochondrial metabolite of crucial importance for energy production by the citric acid cycle, arose from studies demonstrating that patients suffering from coronary heart diseases release amounts cf citrate proportionally correlated with the severity of the disease.In addition, cytosolic citrate is proposed to have a key rote in substrate fuel selection by the heart.The experimental approach s the ex vivo perfused rat heart using 13C-labelled substrates combined with mass isotopomer analysis by gas chromatography coupled te mass spectrometry.Measurements cf citrate efflux in effluent perfusate wete correlated with indices of cardiac performance and metabolic status cf the heart such as the relative contribution cf substrates for citrate synthesis.Altogether, results from the present thesis demonstrate that citrate release from ex vivo perfused rat hearts, which ranges from 5 to 21 nmol.g1.min1 representing < 2% cf the citric acid cycle flux, reflects citrate transfer from the mitochond ria to the cytosol.This process (i) is specific, (ii) is net correlated with indices cf cellular necrosis, (iii) can be modulated by energy demand and exogenous substrate supply for citrate synthesis, (iv) is increased in hypertrophied hearts, and (y) seems to reach a maximal level in the presence cf a physiological substrate mixture, probably due te the low activity cf the mitochondrial and/or plasma membrane transporter.Hence, results from this thesis reinforce the importance cf clarifying the role cf cytosolic citrate as a signal molecule, specifically as the precursor cf malcnyl-C0A, the fatty acid oxidation inhibitor having a major clinical interest.
