Os eventos adversos ao medicamento são uma das principais causas de morbidades e morte nos diversos sistemas de saúde, sendo que, muitos desses eventos adversos ao medicamento resultam de uma fraca comunicação entre profissionais de saúde e o doente durante os cuidados ao doente e no momento da alta. Por outro lado, estas falhas de comunicação com o doente podem acarretar a falta de adesão à terapêutica ou, ao reinício de medicamentos descontinuados, promovendo uma terapia inadequada (Mirco, et al., 2005; Knez et al., 2011). A reconciliação terapêutica é um processo formal no qual os profissionais de saúde trabalham em conjunto com os doentes, familiares e prestadores de cuidados de forma a garantir a transmissão de informação segura e de qualidade acerca da medicação do doente entre as transições de cuidados. Este processo carece de uma revisão sistemática e exaustiva de toda a terapêutica habitual do doente para garantir que os novos medicamentos, os alterados ou os interrompidos, são cuidadosamente avaliados. Este processo é definido como Best Possible Medication History (BPMH) (ISMP Canada, 2012). Por outro lado, após alta hospitalar, os doentes estão vulneráveis aos riscos que resultam da falta de preparação para o autocuidado em casa e de muitas vezes não conseguirem contactar um profissional de saúde que tenha acesso ao seu plano de cuidados quando as dúvidas surgem, e da ausência de seguimento adequado da continuidade do atendimento o que geralmente origina discrepâncias medicamentosas e a um novo internamento. (Cornish et al., 2005) Assim sendo, este trabalho teve como objetivo promover a discussão e reflexão acerca da responsabilidade dos resultados farmacoterapêuticos após alta hospitalar, em sintonia com os princípios da atenção farmacêutica.
