Objectif : l’objectif de cette étude est d’évaluer quantitativement les données issues d’une arthroplastie totale du genou (ATG) assistée par robot avec alignement personnalisé. Le but est de mesurer sa capacité à préserver l’obliquité de la ligne articulaire (JLO), à resserrer et rééquilibrer les genoux opérés, ainsi qu’à resurfacer le fémur distal. Méthodes : il s’agit d’une étude prospective de série continue incluant 100 patients subissant une ATG primaire de mars 2023 à mars 2024, âgés d’au moins 18 ans. Toutes les ATG ont été réalisées à l’aide de la technologie assistée par robot Rosa Knee System avec des implants Persona stabilisés en postérieur (Zimmer Biomet, Montréal, Canada) suivant un alignement personnalisé. L’évolution de la déformation du plan frontal a été mesurée par radiographie en charge. L’évolution de la laxité du genou a été mesurée intra-opératoirement pour chaque patient avant et après l’ATG. La capacité de cette technique à resurfacer le fémur distal a été évaluée à l’aide de scanners avant et après l’ATG en comparant des modèles 3D de chaque patient. Résultats : l’obliquité de la ligne articulaire a été préservée dans 91 % des cas (p < 0,05). Après l’ATG, l’espace fémoro-tibial médial en extension a été réduit en moyenne de 18,6 %. L’espace fémoro-tibial latéral en extension a été réduit de 21 %. L’espace fémoro-tibial médial en flexion a été réduit de 2,6 %. L’espace fémoro-tibial latéral en flexion a été réduit de 23 %. Cette technique a permis de réduire l’asymétrie entre les compartiments médial et latéral de 70 % en extension et de 39,4 % en flexion. La différence moyenne entre la surface articulaire du fémur distal avant et après l’ATG était de 2,27 mm. Conclusion : l’alignement personnalisé lors de l’implantation de l’ATG, tel que décrit dans notre étude, permet de préserver l’orientation de la ligne articulaire dans la majorité des cas grâce à l’assistance robotique. Il permet également de resserrer et rééquilibrer la tension ligamentaire, tout en resurfaçant le fémur distal avec une précision millimétrique.
