Sintomas depressivos ocorrem com grande frequencia e sao potencialmente graves para pacientes com esquizofrenia. Esta tese e composta por estudos avaliando tres aspectos da depressao na esquizofrenia: psicopatologia, nosologia e psicofarmacologia. Os aspectos psicopatologicos foram estudados atraves da traducao da Calgary Depression Scale for Schizophrenia (ADDINGTON et ai. 1990) para o portugues. A Escala Calgary para Depressao em Esquizofrenia foi utilizada em 80 pacientes com esquizofrenia e mostrou alta confiabilidade entre-examinadores, validade concorrente e especificidade. A escala foi considerada um bom instrumento para ser utilizado no Brasil. Os aspectos nosologicos foram estudados atraves da avaliacao da utilidade dos criterios diagnosticos para episodio depressivo em pacientes com esquizofrenia presentes no Manual de Diagnostico e Estatistica dos Transtornos Mentais 4' edicao DSM IV (AMERICAN PSYCHIATRIC ASSOCIATION, 1994) e na Classificacao Internacional das Doencas 10' Edicao - CID 10 (ORGANIZACAO MUNDIAL DA Saúde, 1992) para pacientes com esquizofrenia clinicamente estaveis. O criterio Depressao Pos-Esquizofrenica da CID 10 restringe o diagnostico aos 12 meses que seguem o episodio psicotico agudo, limitando a sua utilidade para pacientes que apresentam episodios depressivos fora deste periodo. O criterio Transtorno Depressivo Pos-Psicotico da Esquizofrenia do DSM IV teve um melhor desempenho sugerindo a incorporacao deste criterio ao eixo I do DSM IV para que este diagnostico possa ser mais amplamente utilizado...(au)
