DETERIORO NEUROCOGNITIVO Y FACTORES ASOCIADOS EN PACIENTES CON INFECCIÓN POR EL VIH

Abstract
Objetivos: Analizar la prevalencia de deterioro cognitivo en una cohorte de pacientes con infección por VIH, en tratamiento antirretroviral estable, utilizando la escala neurocognitiva de Montreal (MoCA) y evaluar los factores asociados.\n\t\t\t\t Material y métodos: Estudio observacional, transversal y descriptivo con componentes analíticos realizado en las consultas externas de Enfermedades Infecciosas del Hospital Reina Sofía en pacientes con infección por VIH que recibieran TAR estable y menores de 60 años. Se utilizó la escala MoCA para evaluar la presencia de deterioro (puntuación < 26). Se consideró deterioro cognitivo severo si la puntuación era inferior a 20. Se realizó un análisis de regresión logística binaria para evaluar los factores asociados a deterioro cognitivo.\n\t\t\t\t Resultados: Se incluyeron 100 pacientes de los cuales un 79% eran varones y con una edad media de 45±9 años. Más de un tercio con antecedentes de consumo de tóxicos, un 30% coinfectados por el VHC y un 39% con síndrome depresivo. Un 44% del total de la muestra presentaron deterioro cognitivo (MoCA < 26), siendo la edad (OR 3,57, IC95% 1,4-9,09; p= 0,007), la depresión (OR 2,8, IC95% 1,1-7,04; p=0,018), y el nivel educativo (OR 2,52; IC95% 1,01-6,3; p = 0,047) las variables que se asociaron de un modo independiente a la presencia de dicho deterioro. El nadir bajo (100 cels/ml) de linfocitos T CD4 se relacionó con la presencia de mayor deterioro cognitivo (MoCA <20) (OR 3,66, IC95% 1-14,01; p=0,047)\n\t\t\t\t Conclusiones: La prevalencia de deterioro cognitivo en pacientes VIH menores de 60 años que reciben TAR estable es elevada. La mayor edad, los episodios depresivos y el nivel educativo bajo fueron factores determinantes. En cambio, el consumo de tóxicos, la coinfección y el tipo de TAR no se asociaron de forma significativa. Por otro lado, el grado de inmunosupresión alcanzado se asoció a un deterioro cognitivo más severo lo que confirma la importancia que tiene el diagnóstico y tratamiento precoz de estos pacientes.
