RÉSUMÉ La douleur chronique (DC) touche une personne sur cinq et à l’heure actuelle, sa gestion demeure sous-optimale dans notre société. Des essais cliniques randomisés sont utilisés pour évaluer l'efficacité des traitements individuels contre la douleur, mais un nombre limité d'études observationnelles ont décrit le soulagement global éprouvé par les personnes vivant avec de la DC. Mon mémoire de maitrise visait à : 1) Mesurer le pourcentage de soulagement de la douleur apporté par les différents traitements utilisés par les personnes vivant avec de la DC et ce, en situation réelle, 2) Identifier les facteurs sociodémographiques et cliniques associés à un meilleur soulagement de la DC. Cette étude transversale exploratoire web a utilisé les données de 1 419 personnes recrutées dans la communauté. Le soulagement global de la douleur apporté par les traitements utilisés par les participants a été évalué à l'aide d'une échelle de 0 à 100 % (par incréments de 10 unités). Un total de 18,2 % des participants ont rapporté un soulagement minimal de la douleur (0-20 %), 60,0 % un soulagement modéré à substantiel (30-60 %) et 21,8 % un soulagement important de la douleur (70-100 %). L'analyse de régression multinomiale multivariable a révélé des facteurs significativement associés à un soulagement plus important de la douleur, notamment le fait de rapporter un événement stressant comme circonstances d’apparition de la douleur, de vivre avec la douleur depuis ≥10 ans, une intensité de douleur plus légère, moins de pensées catastrophiques, l'utilisation de médicaments analgésiques prescrits, l'utilisation de traitements non pharmacologiques, l'accès à un professionnel ou à une professionnelle de la santé de confiance, des scores de santé générale perçue plus élevés et la présence de polypharmacie. Les facteurs associés à un soulagement moindre de la douleur incluaient la chirurgie comme circonstances d’apparition de la douleur, l'utilisation de médicaments analgésiques en vente libre et une détresse psychologique sévère. Dans cet échantillon communautaire de personnes vivant avec de la DC, 8 sur 10 ont déclaré ressentir au moins un soulagement modéré avec leur traitement. L'analyse nous a permis d'explorer des facteurs modifiables potentiels comme des opportunités pour améliorer le bien-être des personnes vivant avec de la DC. ABSTRACT Chronic pain (CP) affects one in five people, and currently, its management remains suboptimal in our society. Randomized clinical trials are used to evaluate the efficacy of various pain treatments individually, while a limited number of observational studies have portrayed the overall relief experienced by persons living with chronic pain. My Master’s degree thesis aimed to describe pain relief in real-world clinical settings and to identify associated factors. This exploratory web-based cross-sectional study used data from 1,419 persons recruited in the community. Overall pain relief brought by treatments used by participants was assessed using a 0-100% scale (10-unit increments). A total of 18.2% of participants reported minimal pain relief (0-20%), 60.0% moderate to substantial pain relief (30-60%), and 21.8% extensive pain relief (70-100%). Multivariable multinomial regression analysis revealed factors significantly associated with greater pain relief, including reporting a stressful event as circumstances surrounding the onset of pain, living with pain for ≥10 years, milder pain intensity, less catastrophic thinking, use of prescribed pain medications, use of nonpharmacological pain treatments, access to a trusted healthcare professional, higher general health scores, and polypharmacy. Factors associated with lower pain relief included surgery as circumstances surrounding pain onset, use of over-the-counter pain medications, and severe psychological distress. In this community sample of persons living with chronic pain, 8 out of 10 persons reported experiencing at least moderate relief with their treatment. The analysis has enabled us to explore potential modifiable factors as opportunities for improving the well-being of persons living with chronic pain.
