Abstract

La pancréatite aiguë (PA) constitue la principale cause d'admissions en centre hospitalier liée à des troubles gastrointestinaux dans le monde, et sa prévalence est en augmentation. La PA est associée à plusieurs comorbidités dont la prévalence mondiale est également en augmentation, comme le syndrome métabolique et le diabète de type 2. L'architecture génétique de la PA demeure toutefois peu caractérisée et peu ou pas d'options thérapeutiques existent présentement pour sa prévention ou son traitement. Notre hypothèse était qu'il est possible d'identifier de nouveaux gènes associés à la PA à l'aide d'études d'association pangénomiques et d'identifier des protéines circulantes associées à la PA à l'aide de la randomisation mendélienne à l'échelle du proteome sanguine humain, et que les résultats de ces analyses pouvaient permettre d'identifier des cibles thérapeutiques potentielles pour la PA. Nos résultats ont non seulement confirmé des associations connues entre certains genes et la PA, mais également mis en évidence de nouveaux gènes potentiellement impliqués dans la maladie. Nous avons également pu identifier plusieurs protéines sanguines causalement impliquées dans la pathophysiologie de la PA dont plusieurs sont des cibles thérapeutiques potentielles pour la PA. En outre, nous avons identifié des médicaments ciblant ces protéines, dont l'effet est cohérent avec celui obtenu grâce aux analyses de randomisation mendélienne et qui pourraient être réutilisés dans le cadre de la prevention ou du traitement de la PA.
