Başlık:KRONİK HASTALIĞI OLAN YAŞLI BİREYLERDE SAĞLIK OKURYAZARLIĞI, SAĞLIK ALGISI VE İLİŞKİLİ FAKTÖRLERTitle:INVESTIGATION OF HEALTH LITERACY, PERCEPTION OF HEALTH AND RELATED FACTORS IN ELDERLY PATIENTS WITH CHRONIC ILLNESSÖzet:Amaç:Araştırma kronik hastalığıolan yaşlı bireylerde sağlık okuryazarlığı, sağlık algısı ve ilişkilifaktörleri incelemek amacıyla yürütülmüştür.Gereç ve yöntem: Tanımlayıcı ve ilişkiselçalışma bir üniversite hastanesinde iç hastalıkları kliniklerinde yataraktedavi gören 65 ve üzeri yaş grubunda gönüllü 550 bireyle yürütülmüştür. Birey tanıtıcı anket formu ve SağlıkOkuryazarlığı Ölçeği kullanılarak yüzyüze görüşme ile toplanmıştır. VerilerSPSS 15 paket programı kullanılarak sayı yüzde dağılımları, Student t testi,Anova varyans analizi, korelasyon, regresyon analizi ve Bonferroni ilerivaryans analizleri ile değerlendirilmiştir. Bulgular: Bireylerin; % 54.9’u kadındır,%69.6’sı evli ve %39.6’sı 65-74 yaş grubunda olup. %34.7’si sağlığını ortadüzeyde, %35.3’ü de yaşam kalitesiniorta düzeyde algılamaktadır. Sağlık hizmetlerine erişmede %51.3’ü çok zorlukçektiğini belirtmiştir. Bireylerin sağlık okuryazarlığı ölçeği puan ortalaması87.96±13.89’dır. Alt ölçeklerin (uygulama, erişim, anlama ve uygulama) maddepuan ortalamaları 17.73±3.28 ile 28.60±5.03 arasındadır. Kadınların erkeklere, 65-74yaş arası bireylerin diğer yaş gruplarına, evli bireylerin bekar olanlara, liseve üzeri eğitim almış grubun daha az eğitim alan gruplara, çalışanlarınçalışmayanlara göre puan ortalaması daha yüksektir. Sağlığını iyi ve çok iyialgılayan, yaşam kalitesini iyi ve çok iyi olarak değerlendiren, sağlık hizmetlerine erişmede hiç zorlukyaşamayan bireylerinsağlık okuryazarlığı puanı daha yüksektir. Kronik hastalık sayısı vekronik hastalık süresi ile sağlık okuryazarlığı ölçeği puan ortalamalarıarasında anlamlı bir ilişki gözlenmedi. Hastaneye yatma sayısı ve acil servisebaşvurma sayısı arttıkça ölçek puanının düştüğü belirlendi. Sonuç: Yaşlıların okuryazarlık düzeyi“az zorluk çekiyorum ile biraz zorluk çekiyorum” düzeyi arasında bulunmuştur. Algılanan sağlık düzeyi ve yaşam kalitesidüzeyi yükseldikçe bireylerin okuryazarlık puanı da artmıştır. Yaş grubu,cinsiyet, eğitim durumu, en uzun yaşanılan yer, sağlık algısı, sağlıkhizmetlerine erişim sağlık okuryazarlığı en çok etkileyen değişkenlerdir. Çalışmanındaha geniş örneklemde yapılması ve hemşirelerin yaşlı bireylere sağlık eğitimi vedanışmanlık etkinliklerinden önce sağlık okuryazarlık durumlarınıdeğerlendirmeleri önerilebilir.Abstract:Aim:The aim was to investigate the health literacy, perceptionof health and related factors in elderly patients with chronic illnesses.Material andmethods: Descriptive and correlational research was carried out with 550 volunteer elderly patients between the ages of65 and over, in a University Hospital. Data was gatheredby using a questionnaire form consisting 30 questions and Health Literacy Scale.Data was evaluated by using SPSS 15 program.Results: 54.9% of the elderly was woman, 69.9% wasmarried, and 39.6% were between the ages of 65-74. Of the elderly 34.7%perceived his/her health at medium level, and 51.3% of them perceived his/her quality of life atmedium level, 51.3% had much difficulty in utilizing health services. Meanscore of health literacy scale was 87.96±13.89 and the mean scores forsubscales were between17.73±3.28 and 28.60±5.03. Those between the ages 65-74,married and those who has a high school level of education and employed has higher scale scores. Mean values of the overall health literacy score and sub-scale scores were higher amongelderly who perceived a good health status and good quality of life and who hasno trouble in utilizing health services. No statistical relation was foundbetween the number of chronical disease, the duration of the disease and scalescore. As the episodes of admission to hospital and to emergency increased, thescale score decreased. Conclusion: Elderly patients’ health literacylevel was between with little difficultyand with some difficulty level. Scale score increased as the level of perceivedhealth status and quality of life increased. Age, gender, status of education,the local area, perception of health and utilization of health services werethe effective factors in health literacy.
