No estamos en la 3ª revolución en salud mental, desarrollo de intervenciones comunitarias, sino en una 4ª, la prevención y promoción de salud.. La sociedad globalizada y liberal tiene un estado menos controlador que también da menos apoyo. Aparecen además nuevas y numerosos desórdenes emocionales que requieren atención. La dificultad de extender la promoción de la salud en la prestación de servicios deviene del modelo explicativo anclado en lo individual y en la enfermedad además de la dificultad para definir el concepto promoción de la salud. Lo que se ha hecho en la declaración de Ottawa(1986). En la Región sudamericana se detecta un cambio de perfil epidemiológico y nuevos grupos de riesgo. Estábamos en los comienzos de la 3ª revolución y ya está en el centro de la atención la 4ªque la Declaración de Caracas se ha comprometido a impulsar. La colaboración internacional del centro colaborador de Granada (España) a modo de consultoría avanza en el diseño de un plan de modernización y redefinición organizativa de los servicios y redes asistenciales. Describimos sucintamente la situación actual de la red y subprogramas en desarrollo en Chile.
