Abstract
Introducción: El diagnóstico tardío del cáncer de cérvix favorece el desarrollo de complicaciones como la uropatía obtructiva. La nefrostomía se hace necesaria para resolver la disfunción renal, sin embargo, su indicación y beneficios siguen siendo controversiales. \nObjetivo: Identificar la morbimortalidad asociada a la nefrostomía en pacientes con uropatía obstructiva por cáncer de cérvix en el Instituto Nacional de Cancerología durante los años 2012- 2016. \nMateriales y métodos: Se realizó un estudio de cohorte bidireccional para caracterizar la población, hacer seguimiento y establecer la mortalidad, morbilidad y factores asociados. \nResultados: Se incluyeron 100 mujeres con cáncer de cérvix y nefrostomía, el 50% se diagnosticaron en estadios IIIB y 42% de los casos presentó metástasis al momento del procedimiento. Se hospitalizó el 57% de las pacientes con un promedio de estancia de 20,71 días; 60% de las pacientes presentó algún tipo de complicación. La prevalencia de infección urinaria fue del 41% y la de mortalidad del 52%; la mediana de supervivencia general fue de 10,9 meses. La principal causa de fallecimiento fue progresión de la enfermedad. La hipoalbuminemia y la IVU mostraron ser posibles factores para mal pronóstico. \nConclusiones: La mortalidad de las pacientes sometidas a nefrostomías y las complicaciones asociadas al procedimiento disminuyeron con respecto al año 2005. De igual forma la prevalencia de infecciones urinarias asociada a dispositivo fue menor, sin embargo, hubo cambios en el perfil microbiológico.
