Tendinopatia e dolore: il ruolo della neuroplasticitá nel percorso riabilitativo. Revisione sistematica della letteratura

Abstract
Introduzione: \nLa tendinopatia è una patologia dei tendini caratterizzata da dolore, deterioramento della funzione e ridotta \ntolleranza all'esercizio. Recentemente, si è iniziato a considerare il ruolo del sistema nervoso centrale (SNC) \nnella comprensione del dolore associato alle tendinopatie. \nObiettivo: \nL’elaborato è focalizzato sull’influenza dei fenomeni di sensibilizzazione del sistema nervoso centrale e di \nneuroplasticità in relazione al ruolo del dolore nelle tendinopatie. \nInoltre, è indirizzato verso l’individuazione dell’approccio terapeutico che ad oggi risulta essere il più efficace \nper la risoluzione della sensibilizzazione del dolore nei soggetti con tendinopatia. \nMateriali e metodi: \nSono state seguite le linee guida di “Preferred Reporting Items for Systematic reviews and Meta-Analyses” \n(PRISMA 2020). Gli articoli pertinenti sono stati identificati attraverso una ricerca informatizzata su PubMed, \nCohcrane, PEDro e Scopus. La valutazione della qualità metodologica è stata effettuata mediante le scale \nNewcastle-Ottawa e AMSTAR2. Sono stati analizzati 5 studi di cui 2 Revisioni Sistematiche con Metanalisi, \n1 Revisione Sistematica, 1 studio Trasversale e 1 studio Caso-controllo. \nRisultati: \nLa ricerca ha prodotto 411 risultati e tramite un processo di selezione sono stati inclusi 5 articoli completi che \nsoddisfano i criteri di inclusione. Tre studi sottolineano la presenza ed un coinvolgimento del sistema nervoso \ncentrale in soggetti con tendinopatia mentre i rimanenti due studi riscontrano evidenze contrastanti. \nConclusioni: \nLa revisione suggerisce che la sensibilizzazione centrale del dolore e i fenomeni di neuroplasticità svolgono \nun ruolo da tenere in considerazione nelle tendinopatie; questi meccanismi, infatti, possono contribuire alla \npersistenza del dolore, alla compromissione della funzione e al peggioramento della qualità della vita dei \npazienti.
