Abstract
Amaç: Çalışma total kalça protezi ameliyatı sonrası verilen videolu taburculuk eğitiminin fonksiyon, günlük yaşam aktiviteleri ve yaşam kalitesi üzerine etkisini incelemek amacıyla planlandı. Yöntem: Çalışmaya total kalça protezi ameliyatı geçirmiş otuz bir hasta (ortalama yaş = 52,9 ± 11,12 yıl, 21 kadın, 10 erkek) dahil edildi. Hastalar rehabilitasyon (n=18) ve eğitim grubu (n=13) olarak rastgele iki gruba ayrıldı. Rehabilitasyon ve eğitim grubuna cerrahi sonrası hastane odasında fizyoterapi programı uygulandı. Eğitim grubuna fizyoterapi programına ek olarak aynı zamanda birebir gösterim ile video tabanlı eğitim verildi. Hastaların ağrı algı düzeylerini belirlemek için Görsel Analog Skalası (GAS), kalça fonksiyon ölçümleri için Harris Kalça Skoru ve Western Ontario ve McMaster Üniversiteleri Osteoartrit İndeksi (WOMAC), fiziksel yeteneği değerlendirmek için 40 Metre Hızlı Tempo Yürüme Testi, hareket/tekrar yaralanma korkusu ve korku-kaçınma parametrelerini ölçmek amacıyla Tampa Kinezyofobi Ölçeği, yaşam kalitesini değerlendirmek için Notthingham Genişletilmiş Günlük Yaşam Aktiviteleri İndeksi kullanıldı. Memnuniyet düzeyi tarafımızca hazırlanan 5 soruluk anket formları kullanılarak sorgulandı. Değerlendirmeler cerrahi sonrası 1. haftada ve 3. ayda yapıldı. Bulgular: Rehabilitasyon ve eğitim grubu karşılaştırıldığında istirahat ve aktivite sırasında ağrıda, kalça fonksiyonunda Hariss Kalça Skorunda ağrı ve WOMAC indeksinde sertlik alt parametresi dışında eğitim grubu lehine istatistiksel olarak anlamlı fark saptandı (p<0.05). 40 m hızlı tempo yürüme testinde anlamlı fark bulunmazken (p>0.05), korku-kaçınma düzeyinde eğitim grubu lehine anlamlı fark bulundu (p<0.05). Yaşam kalitesinin sadece hareket alt basamağında eğitim grubu lehine anlamlı fark var ilen (p<0.05), mutfak aktiviteleri, ev işleri, boş zaman aktiviteleri, toplam skor alt basamaklarında istatistiksel olarak anlamlı fark yoktu. Memnuniyet sonuçlarının tüm alt basamaklarında eğitim grubunda anlamlı derecede yüksek olduğu gözlendi. Sonuç: Total kalça protezi ameliyatı sonrası verilen videolu taburculuk eğitiminin fonksiyon ve korku kaçınma düzeyi üzerine olumlu etkilerinin olduğu ve hastaların bilgi ve memnuniyet düzeylerini arttırdığı görüldü.
