Individuals are different they need to be cared for according to where they are at in their recoveryThis thesis is a final project to the completion of a B.Sc. degree in nursing from the University of Akureyri.The aim of the proposed study is to explore the experiences of patients with heart failure, from a person centered care approach, and support for self-care in the outpatient clinic at Landsptali Hospital (LSH).It is believed that the number of heart failure cases will increase in coming years due to increased age in the community, as well as an increase in other chronic diseases.The study will focus on individuals with heart failure who use outpatient services for heart failure at LSH. Person centered care is a concept that must be more carefully put into practice in the health care services.Seeing as individuals are different they need to be cared for according to where they are at, and the person behind the illness needs to be considered by giving their experiences and preferences priority.A person centered care approach emphasizes the patient's active participation in recovery, it is thus useful when motivating individuals towards self-care.Self-care is a complex process that involves the recognition of individual symptoms, and assessing treatment.We utilized the Vancouver school of phenomenology.Data was collected through individual interviews with patients who suffered from heart failure and had utilized the services of the outpatient clinic for heart failure and the participants were 20 patients.The authors hope that the results of the proposed research will strengthen the concept of the person centered care approach to health care, and that it creates awareness among health care providers, which in turn may lead to a more attentive approach when motivating individuals to better self-care.
