Abstract
Objectifs : L’objectif principal consistait à décrire la documentation des interventions des pharmaciens aux dossiers des patients inscrits à l’urgence ou admis à l’Institut universitaire de cardiologie et de pneumologie de Québec – Université Laval, ciblés selon l’offre de soins pharmaceutiques actuelle. \n \nMéthode : Cette étude descriptive rétrospective transversale a examiné la documentation aux dossiers des patients analysés par les pharmaciens en fonction des principes de l’offre de soins pharmaceutiques en vigueur du 5 au 21 décembre 2018. L’étude porte sur le type de notes, la nature des interventions consignées et l’endroit de documentation. \n \nRésultats : Sur 1267 dossiers révisés ponctuellement par un pharmacien, 753 (59,4 %) étaient documentés et 94 (7,4 %) comportaient au moins une note de prise en charge globale. Les évaluations pharmaceutiques et les activités de surveillance représentaient 79,7 % des interventions documentées. Le dossier médical était l’unique support de consignation dans 65,7 % des cas, le dossier pharmacologique dans 23 % des cas et 11,3 % des interventions étaient inscrites dans les deux supports. Une proportion de 67,3 % des interventions consignées dans le dossier médical apparaissait dans les notes évolutives, et 64,8 % de celles du dossier pharmacologique figuraient dans le champ « LS » de GesphaRxMD. \n \nConclusion : Cette étude, décrivant la documentation dans le cadre d’une offre de soins pharmaceutiques hiérarchisant la prise en charge des patients selon leur vulnérabilité, révèle un écart entre la pratique attendue et ce qui se fait sur le terrain. Il s’agit d’une première étape vers l’optimisation de cette activité.
