Dissociação entre funções executivas e processamento comunicativo: um estudo de caso pós-traumatismo cranioencefálico

"Estudos de caso destacam-se por possibilitar a compreensão clínica na medida em que exploram associações e dissociações do desempenho de acordo com o padrão esperado. As dificuldades de comunicação, especialmente no nível do discurso, são queixas relatadaspor pacientes pós-traumatismo cranioencefálico (TCE). Não há consenso, na literatura, se esses déficits são primários de ordem comunicativa ou secundários a outros déficits cognitivos, tais como os executivos. Esse estudo de caso buscou verificar se existem associações ou dissociações entre o processamento discursivo e executivo em um paciente pós-TCE leve. Na bateria de avaliação neuropsicológica, foi utilizado os testes NEUPSILIN, Bateria Montreal de Avaliação da Comunicação, Wisconsin, Hayling, Span Auditivo de Palavras em Sentenças, Mini Exame do Estado Mental, Teste das Trilhas, Teste de Aprendizagem Auditivo-Verbal de Rey, e Bateria MAC Breve priorizando-se uma bateria com enfoque na funcionalidade e no perfil neuropsicológico. Observaram-se dissociações entre os processamentos discursivo e executivo, além de dissociações intraprocessamento executivo. De modo geral, a paciente apresentou maior dificuldade em testes menos ecológicos com demanda executiva complexa de velocidade de processamento, inibiçãoe planejamento, nas tarefas discursivas, apresentou desempenho não sugestivo de déficits, no entanto, com características de um discurso tangencial, desorganizado e confuso, porém não suficientes para padrão deficitário. Hipotetiza-se que a linguagem tenha compensado os déficits cognitivos quando as tarefas exigiam mais da comunicação, confirmando nesse sentido, disfunção executiva predominante para tarefas de alta complexidade. Além disso, o alto nível de escolaridade e o tempo pós-lesão de dez meses podem ter sido bons preditores para compensar os déficits funcionais apesar do dano cerebral. Sugere-se a realização de estudos de casos múltiplos e comparação com clusters conforme desempenho comunicativo versus executivo, a fim de comparar o desempenho de pacientes com diferentes gravidades de lesão e/ou hábitos de leitura."
