La fibrilación auricular es la arritmia cardíaca más frecuente en la población anciana. Dos tercios de los casos de fibrilación auricular ocurren en los pacientes mayores de 75 años y 20% ocurre en mayores de 85 años; 5 por lo que es el grupo vulnerable a la mayor incidencia de eventos cerebrovasculares de tipo cardioembólico. Tenemos fuerte evidencia del efecto beneficioso del tratamiento anticoagulante oral en la prevención de las complicaciones del accidente cerebrovascular cardioembólico y otros fenómenos tromboembólicos. Sin embargo, la terapia de anticoagulación es deficiente en el paciente. Se considera que el presente trabajo es de gran relevancia debido a la escasa información local sobre los factores que determinan en nuestra práctica clínica diaria la decisión de no anticoagular en este grupo etario, permitiéndonos así identificar variables potencialmente modificables que promuevan el uso óptimo de la terapia en el tratamiento de la fibrilación auricular, contribuyendo en la elaboración de los protocolos en el servicio de geriatría. De esa forma contribuir a la baja de las complicaciones tromboembólicas, causando un impacto en la funcionalidad del paciente adulto mayor, asimismo se verá reflejado en la esfera económica a nivel de la institución disminuyendo los costos de las mismas.
