Les évidences observationnelles suggèrent une association entre la consommation de produits laitiers et une diminution du risque de diabète de type 2. Néanmoins, les études cliniques demeurent controversées, probablement en raison de la grande variabilité interindividuelle en réponse aux produits laitiers. De plus, très peu d’études utilisent des biomarqueurs objectifs pour évaluer la consommation de produits laitiers. L’analyse sérique des profils de métabolites avec la métabolomique pourrait être une stratégie intéressante d’identification de biomarqueurs de la consommation de produits laitiers et ainsi, améliorer la précision des évaluations nutritionnelles dans les études cliniques. L’objectif global de ce mémoire est d’observer l’effet d’une consommation augmentée de produits laitiers sur les différents paramètres glycémiques et insulinémiques associés au diabète de type 2, de même que sur les profils métabolomiques chez des sujets hyperinsulinémiques. De plus, un objectif spécifique est d’évaluer la réponse interindividuelle en réponse aux produits laitiers. La réalisation d’un essai randomisé en chassé-croisé entrepris auprès de 27 adultes hyperinsulinémiques suggère qu’une consommation de ≥4 portions/jour de produits laitiers pendant 6 semaines n’affecte pas les différents paramètres glycémiques et insulinémiques comparativement à ≤2 portions/jour. Néanmoins, augmenter à ≥4 portions/jour la consommation de produits laitiers semble accentuer la résistance à l’insuline comparativement à avant l’intervention. Toutefois, une grande variabilité dans la réponse interindividuelle fut remarquée pour la résistance à l’insuline, suggérant qu’une consommation augmentée de produits laitiers pourrait être favorable ou défavorable selon les individus. De plus, une consommation de ≥4 portions/jour affecte les profils de métabolites de façon distincte à ≤2 portions par jour, suggérant une signature de métabolites spécifique au nombre de portions consommées. Davantage d’études de longue durée tenant compte de la variabilité interindividuelle et utilisant des biomarqueurs objectifs sont requises afin d’identifier les individus qui pourraient réellement bénéficier d’une augmentation de leur consommation produits laitiers pour prévenir le diabète de type 2.
