Lomber spinal stenoz (LSS) lomber omurganın nöral ve vasküler dokularını çevreleyen yapıların daralması sonucu görülen, kalça ve alt ekstremite ağrısına neden olan klinik durumdur. LSS tanısında radyolojik ve elektrofizyolojik yöntemler kullanılmaktadır. Paraspinal denervasyonun kliniği görüntüleme yöntemlerinden daha iyi yansıttığı düşünülmektedir. Lomber ekstansiyon ile bel ve bacak ağrıları tetiklenen hastalar zamanla öne eğik postür edinirler. Öne eğik postür, kişinin vücut ağırlık merkezinin yer değiştirmesine neden olur. Bu durum, denge bozukluklarına ve düşmelere neden olabilir. LSS her zaman klinik olarak bulgu vermeyebilir. Bu çalışmada LSS tanılı hastalarda paraspinal kas denervasyonu ve paraspinal kas denervasyonunun denge üzerine etkilerini araştırmayı amaçladık. Çalışmanın ikincil amacı LSS tanılı hastalarda en fazla etkilenen denge parametresini saptamak ve paraspinal haritalama (PSH) skoru ile aralarındaki ilişkiyi belirlemekti. \nÇalışmaya Marmara Üniversitesi Hastanesi Fiziksel Tıp ve Rehabilitasyon Polikliniği’ne başvuran hastalardan klinik olarak LSS tanısı alan ve semptomu olmasa bile radyolojik olarak LSS saptanmış hastalar alındı. Hastalar üç aşamada değerlendirildi. Birinci klinisyen tarafından hastaların fonksiyonel durumu Modifiye Oswestry Özürlülük İndeksi ve Roland Morris Sorgulama Formu ile, ağrı düzeyleri McGill Ağrı indeksi ile ölçüldü. İkinci bir klinisyen tarafından sinir iletim çalışmaları, alt ekstremite iğne EMG ve PSH yöntemi ile elektrofizyolojik yöntemler uygulandı. Daha sonra hastalar üçüncü bir klinisyen tarafından denge için; Berg Denge Ölçeği (BDÖ), zamanlı kalkma ve yürüme testi ve Neurocom Balance MasterTM denge cihazı ile değerlendirildi. \n40-75 yaş arası 37 hasta (klinik:19, radyolojik: 18) iki gruba ayrıldı. PSH skorları (p=0,001), Modifiye Oswestry Özürlülük İndeksi (p=0,001), Roland Morris Sorgulama Formu (p=0,001), ve McGill Ağrı İndeksi (p=0,001), klinik LSS grubunda anlamlı olarak daha yüksek bulundu. Berg Denge Ölçeği skoru klinik LSS grubunda anlamlı olarak daha düşük saptandı (p=0,001). Zamanlı kalkma ve yürüme testi açısından iki grup arasında anlamlı fark saptanmadı (p=0,561). Tüm hastalar değerlendirildiğinde PSH skoru ile en yüksek korelasyon BDÖ arasında saptandı (r=-0,669, p=0,001). PSH skoru arttıkça BDÖ skorunda azalma olduğu görüldü. Gruplar kendi içinde değerlendirildiğinde radyolojik LSS grubunda ağrının da dengeyi PSH skorundan bağımsız olarak etkileyebildiği saptandı. Klinik LSS grubunda ise PSH skoru ağrıdan bağımsız olarak denge parametrelerini olumsuz etkilemekte idi. BDÖ skoru sonuçları ile denge cihazından elde edilen değerler uyumlu idi. \nBDÖ ile en yüksek korelasyon gösteren parametre stabilite sınırları testinde 5 numaralı hedefe yönelimde son nokta hareketi idi (r=0,742, p=0,001). \nSonuç olarak; paraspinal kas denervasyonunun dengeyi bozan bir etkendir. PSH skorunun artması Berg Denge Ölçeği ve denge cihazı ile yapılan değerlendirme sonuçlarını ağrıdan bağımsız olarak olumsuz etkilemektedir. Berg Denge Ölçeği’nin denge değerlendirme amacıyla kullanılabilecek uygun ve güvenilir bir yöntemdir. Berg Denge Ölçeği, denge cihazındaki testlerle uyumlu sonuç vermektedir. Denge cihazı ile yapılan değerlendirme ayrıntılı bilgi verir ancak oldukça zaman alıcıdır. En anlamlı parametreler; stabilite sınırları testinde saptanmıştır. Klinikle de uyumlu olarak LSS hastalarının yürüme hızları yavaş, adım uzunluğu kısa ve simetrisi bozuktur. Hastaların lomber ekstansiyon içeren hareketlerde yön kontrolü, stabiliteleri ve ağırlık merkezini ilerletme mesafeleri bozulmaktadır. \nAnahtar sözcükler: Lomber spinal stenoz, paraspinal haritalama, denge. \n\nABSTRACT \nLumbar spinal stenosis (LSS) is a clinical syndrome as a result of constricted structures around the neural and vascular tissues of the spine. Radiologic and electrophysiologic methods are utilized to diagnose LSS. Paraspinal muscle denervation is thought to be more relevant to clinic rather than imaging techniques. Back and radiating leg pain are generally triggered by lumbar extension. Therefore, patients adopt a bent back or simian posture in the course of time. Bent back posture leads to displaced center of gravity (COG). Sometimes, it may not be possible to diagnose LSS clinically. In this study our aim is to identify paraspinal muscle denervation and its effects on balance among patients having LSS. Secondary goal is to figure out the most affected parameter of balance and its correlation with paraspinal mapping (PSM) scores in LSS patients. \nPatients, having admitted to Marmara University Medical School Physical Medicine and Rehabilitation Outpatient Clinics and diagnosed with LSS either clinically or radiologically, were enrolled into study. Patients were evaluated in three steps. The first physician assessed patients with Modified Oswestry Disability and Roland Morris Questionnaire in terms of functionality and with McGill Pain Index in terms of pain. The second physician performed nevre conduction studies and needle EMG of the lower limbs and PSM for all patients. Then, patients were evaluated by another physician in terms of balance with Berg Balance Scale (BBS), timed-up and go test (TUGT) and Neurocom Balance MasterTM. \n40-75 year-old 37 patients (clinic LSS: 19, radiologic LSS: 18) were assigned into two groups. PSM scores (p=0,001), Modified Oswestry Disability (p=0,001), Roland Morris Questionnaire and McGill Pain Index (p=0,001) were significantly higher in clinic LSS group. BBS scores were significantly lower in clinic LSS group (p=0,001). There was no difference in TUGT between the groups (p=0,561). Among all the patients, BBS scores were the highest correlated parameter to PSM scores(r=-0,669, p=0,001). BBS scores were realized to be diminished in patients having higher PSM scores. Regardless of PSM scores any pain, even not due to LSS, may affect the balance in radiologic LSS group. On the other hand, PSM scores affected balance negatively, regardless of pain level in clinic LSS group. Results of balance device tests were in consistence with BBS. When balance tests compared to BBS, end point excursion 5 at limits of stability (LOS) test had the highest correlation with BBS (r=0,742, p=0,001). \nIn conclusion; paraspinal muscle denervation affects balance negatively. Increased PSM scores lead to draw back in BBS and balance device test results. BBS is convenient to the assessment with balance device. BBS is an appropriate and safe method to evaluate balance. Assessment of balance with devices provide more detailed data, but this is a time-taking method and not easy to perform within daily routine. The most significant parameters of balance device were obtained from LOS test. In consistence with the clinic, paraspinal muscle denervation ends up with walking more slowly in shorter and more asymmetric steps. Patients have difficulty in maintaining their directional control, stability and COG excursion during activities including extension of the lumbar spine.
