Abstract
L'objectif de la thèse était d'évaluer les liens entre propriétés biomécaniques et imagerie rachidienne. A cette fin, un protocole expérimental in vitro a été mis en oeuvre sur 22 disques intervertébraux lombaires humains. Des essais caractérisant la mobilité sous charge, la viscoélasticité et la réponse dynamique des disques ont été réalisés, tandis que des radiographies, discographies et coupes IRM ont été recueillies. Les disques ont fait l'objet d'analyses macroscopiques et chimiques. Des modèles rhéologiques ont été développés. Enfin, des images radiographiques en double énergie ont été reconstruites. Trois grandes classes de caractéristiques ont été extraites des images: les grades de dégénérescence issus de trois modalités différentes, la géométrie et les singularités pathologiques. Pour chaque sollicitation, leurs effets ont été recherchés. Les anomalies de type scoliose ou calcifications de grande taille affectaient fortement la mobilité. Après exclusion des spécimens les plus pathologiques, des relations entre amplitudes de mobilité et géométrie (hauteur et surface) ont pu être mis en évidence. Fluage, relaxation et hystérésis étaient liés, et principalement affectés par la dégénérescence. Peu de caractéristiques disponibles influaient le comportement dynamique. Enfin, le numéro atomique équivalent du disque intervertébral a pu être mesuré grâce à la radiographie en double énergie, et partiellement validé par l'analyse chimique. Ainsi cette thèse a permis de quantifier, à l'aide de l'imagerie, certains aspects du comportement biomécanique du disque intervertébral. \n \n
