Introdução: Durante décadas, a avaliação do ventrículo direito (VD) foi ignorada pelo facto de este ser considerado irrelevante para a função cardíaca. Só na segunda metade do século passado é que o VD viu reconhecido o seu papel fundamental em várias condições fisiológicas e patológicas. No entanto, a avaliação sistemática da função cardíaca direita não é ainda realizada de forma uniforme devido, em grande parte, à maior atenção dada à avaliação do coração esquerdo e à falta de familiaridade com as técnicas ultrassonográficas que podem ser aplicadas ao estudo da imagiologia do coração direito. O objectivo geral deste trabalho de investigação consistiu em descrever e correlacionar os valores da Função Sistólica do Ventrículo Direito, obtidos por diferentes parâmetros de quantificação em Ecocardiografia Transtorácica, em indivíduos adultos sem patologia cardiorespiratória conhecida. Metodologia: Estudo prospectivo do tipo descritivo-correlacional com uma abordagem quantitativa, implementado em meio laboratorial. População: indivíduos adultos sem patologia cardiorespiratória conhecida que realizaram Ecocardiograma Transtorácico no Centro Cardiovascular do Hospital da Luz entre Março e Junho de 2011. Amostra: quatrocentos indivíduos adultos seleccionados através de métodos de amostragem não probabilística do tipo acidental. Variáveis: variáveis ecocardiográficas para estudo da função sistólica ventricular direita, variáveis ecocardiográficas caracterizantes do VD e variáveis atributo. Resultados: Foram estudados quatrocentos indivíduos com idade média de 48 anos, 59% do sexo feminino, superfície corporal média de 1,78m2 e Índice de Massa Corporal (IMC) médio de 25,19Kg/m2. A Fracção de Encurtamento da Câmara de Saída do VD média de 63%, a Excursão Sistólica do Plano do Anel Tricúspide (TAPSE) média de 23mm, Velocidade da onda S do anel tricúspide por Doppler Tecidular (TDI) média de 14cm/s, Índice Performance Miocárdica (IPM) médio de 0,29, Fracção da Área de Encurtamento média de 56%, Strain 2D médio da parede lateral basal de -28%, da parede lateral mediana de -28% e da parede lateral apical de -22%, Strain Rate 2D médio da parede lateral basal de -2,17s-1, da parede lateral mediana de -1,81s-1 e da parede lateral apical de -1,64s-1, Strain TDI médio da parede lateral basal de -27%, da parede lateral mediana de -30% e da parede lateral apical de -28% e Strain Rate TDI da parede lateral basal de -1,49s-1, da parede lateral mediana de -1,64s-1 e da parede lateral apical de -1,57s-1. Verificou-se a presença de correlações lineares moderadas e uma correlação linear forte (r = 0,796, p= 0,000) entre os parâmetros de deformação miocárdica do VD obtidos por 2D-Speckle Tracking e por Doppler Tecidular bem como uma correlação linear moderada entre a TAPSE e a velocidade da onda S do anel tricúspide obtida por Doppler Tecidular. O parâmetro que não apresentou correlação linear com qualquer um dos outros parâmetros foi o IPM. Considerações finais: Todos os valores de função sistólica ventricular direita obtidos pelos diferentes parâmetros avaliados apresentaram resultados que se encontram dentro dos intervalos definidos pelas Guidelines for the Echocardiographic Assessment of the Right Heart in Adults das Sociedades Americana, Europeia e Canadiana de Ecocardiografia. Com este trabalho depreende-se que os parâmetros ecocardiográficos de avaliação da função ventricular direita são relativamente independentes entre si e que características como a idade, o sexo ou a classe de IMC não modificam significativamente a intensidade da correlação linear dos parâmetros ecocardiográficos. ABSTRACT - Introduction: In the past decades the right ventricle (RV) evaluation was ignored because it has been considered irrelevant for heart function. It was only recognized the important physiological and pathological role of RV in the second half of the last century. However, the evaluation of the right cardiac function is not systematically performed, due to the higher importance that is recognized to the left heart and to the lack of ultrasound techniques that can be applied to the imaging study of the right heart. The main aim of this study was to describe and to correlate the right ventricular systolic function values, obtained by different quantitative parameters in transthoracic echocardiography of healthy adults without cardiopulmonary pathology. Methods: It is a prospective descriptive correlational study, using a quantitative approach, implemented in a laboratorial environment. Population: adult individuals without cardiopulmonary pathology that performed an transthoracic echocardiography at Cardiovascular Center, Hospital da Luz, from March to June 2011. Sample: 400 adults selected by an accidental nonprobabilistic sampling technique. Variables: Echocardiographic variables for the study of the right ventricular systolic function, characteristic Echocardiographic variables of the RV and variable attributes. Results: A sample of 400 individuals was studied with an average age of 48 years old, 59% were female, presenting an average body surface area of 1,78m2 and a body mass index (BMI) of 25,19Kg/m2. The average Right Ventricular Outflow Tract Shortening Fraction was 63%, while the Tricuspid Annular Plane Systolic Excursion (TAPSE) was 23mm. The average Systolic Wave Velocity of the Tricuspid Annular Plane measured by Tissue Doppler Imaging (TDI) was 14cm/s; The average Myocardial Performance Index (MPI) was 0,29, the average Fractional Area Change was 56%, the average two-dimensional (2D) peak Strain of the basal lateral wall was -28%, the lateral mid wall was -28% and the lateral apical wall was -22%. The average 2D peak Strain Rate of the lateral basal wall was -2,17s-1, of the lateral mid wall was -1,81s-1 and of the lateral apical wall was -1,64s-1. The average Doppler peak Strain of the lateral basal wall was -27%, of the lateral mid wall was -30% and of the lateral apical wall was -28%. The average Doppler peak Strain Rate of the lateral basal wall was 1,49s-1, of the lateral mid wall was -1,64s-1 and of the lateral apical wall was -1,57s-1. Moderate and strong (r=0,796, p=0,000) linear correlations were observed between parameters of the RV myocardial deformation obtained by 2D Speckle Tracking and TDI. In addition, a moderate linear correlation was observed between TAPSE and the Systolic Wave Velocity of the Tricuspid Annular Plane. Interestingly, MPI did not present any linear correlation with any of the studied parameters. Conclusion: The data on the right ventricular systolic function obtained by the different echocardiographic methods was located within the range defined by the Guidelines for the Echocardiographic Assessment of the Right Heart in Adults, from the American, European and Canadian Societies of Echocardiography. As a result of this work, the echocardiographic parameters for the evaluation of the RVF were found to be relatively independent. In addition, parameters such as age, gender or BMI did not significantly alter the intensity of the linear correlation of the echocardiographic parameters under study.
