Abstract
Base Teórica: A artrite reumatoide (AR) é uma doença autoimune inflamatória crônica que leva a dano e incapacidade articular. Está associada a um aumento da mortalidade e de custos socioeconômicos. Pacientes com AR podem desenvolver várias complicações musculoesqueléticas como perda muscular, quedas e fratura em relação a população saudável. A sarcopenia é uma condição multifatorial, com várias causas associadas além da idade, sua etiologia envolve fatores genéticos e ambientais, dependentes de reguladores positivos e negativos do crescimento muscular. A sarcopenia está associada a perda muscular progressiva, estando relacionada a fratura por fragilidade. A fratura osteoporótica afeta a sobrevida e a qualidade de vida dos pacientes com AR, sendo a coluna vertebral o local mais comum de fratura em idosos com osteoporose. As fraturas podem ser muito debilitantes, especialmente em pacientes idosos. O risco de fratura vertebral e não vertebral é o dobro em pacientes com AR. Objetivo: Estimar a prevalência de fraturas em pacientes com artrite reumatoide com sarcopenia através de uma revisão sistemática da literatura. Métodos: Uma revisão sistemática de estudos observacionais foi conduzida usando MEDLINE (via PubMed), Embase, LILACS, Web of Science. As estratégias de pesquisa foram baseadas em palavras-chave predefinidas e títulos de assuntos médicos. A triagem de título, resumo e texto completo foi realizada em duplicata por dois revisores independentes. A qualidade metodológica dos estudos foi avaliada pela escala Newcastle-Ottawa adaptada e os estudos foram incluídos independentemente da qualidade metodológica calculada. Resultados: Foram identificados 968 artigos potencialmente relevantes com base na estratégia de pesquisa. Após a triagem de título e resumo 59 artigos foram incluídos para triagem de texto completo e dentre esses 57 artigos foram excluídos de acordo com os critérios de inclusão e exclusão. Portanto, 2 artigos completos foram incluídos na revisão sistemática. O resultado dessa revisão sistemática indica que a prevalência de fraturas em pacientes com artrite reumatoide com sarcopenia varia entre 12,5% a 25,9%. Conclusão: A prevalência de fraturas em pacientes com artrite reumatoide com sarcopenia varia entre 12,5% a 25,9%. A revisão sistemática sugere que a maior parte das fraturas nos pacientes com artrite reumatoide ocorre nos pacientes com sarcopenia.
