RÉSUMÉ : Contexte : Les catastrophes naturelles associées aux changements climatiques sont plus courantes avec l’augmentation des gaz à effet de serre. Leurs impacts sont multiples sur le système de santé et ils représentent un défi actuel et à long terme important pour la santé et le bien-être des populations touchées. L’objectif de ce mémoire est d’examiner dans quelle mesure il pourrait être souhaitable de définir et de déployer un projet de cadre de gestion intégrée des risques permettant de mieux prendre en considération les risques induits par les changements climatiques sur la santé mentale au Québec. Méthodologie : un examen de la portée (scoping review) a été effectué pour explorer comment la gestion des risques est abordée dans les services de santé et les services sociaux à l’échelle mondiale avant la pandémie de COVID-19, entre 2010 et 2020. Ensuite, un examen des rapports annuels de gestion et financiers des établissements de santé au Québec a été effectué pour explorer si et comment cette gestion du risque figure parmi les priorités d’action des différents établissements de santé au Québec avant la pandémie de COVID-19, entre 2015 à 2020. Résultat : Au total, 15 écrits provenant des pays de l’OCDE ont été analysés dans l’examen de la portée (scoping review). La majorité des écrits (10/15) retenus sont des articles de synthèse (revue systématique ou examen de la portée) ou des articles de perspective (ou d’opinion) évalués par les pairs. Aucun écrit sur la gestion des risques induits par les changements climatiques sur la santé mentale est abordée auprès des autres services de santé et services sociaux à l’échelle mondiale n’a été repéré. Les écrits abordent les thématiques suivantes : la proposition de cadre de référence sur la gestion des risques des changements climatiques ; l’appréciation de l’expérience vécue de catastrophes naturelles chez la population ; et l’exploration des risques, des vulnérabilités, des impacts et des déterminants de la santé. Ces résultats demeurent néanmoins intéressants puisqu’ils peuvent orienter, guider et soutenir les décideurs dans la gestion des risques en santé mentale et permettre l’identification des processus lacunaires ou les actions que les établissements de santé doivent renforcer pour accroître leur résilience aux changements climatiques. Les 325 rapports annuels de gestion ainsi que les rapports financiers des CISSS, CIUSSS, CHU, instituts du Québec et le MSSS avant la pandémie de COVID-19 ont été analysés, entre la période de 2015 à 2020. Vingt-six mentions d’un budget spécifique pour les inondations dans les rapports financiers des établissements de santé ont été repérées. Parmi ceux-ci, trois mentions sont faites pour un volet psychosocial à la sécurité civile pour les inondations. Nous retrouvons deux mentions en lien avec un budget sur les changements climatiques dans les rapports financiers de deux CISSS. Cependant, les rapports financiers n’élaborent pas le détail de ces budgets et il est difficile de retracer les informations détaillées dans les rapports annuels de gestion. Seize mentions aux changements climatiques proviennent de 9 % (3/33) des établissements de santé. Ils sont le MSSS, les CISSS de la Côte-Nord et le Conseil Cri de la santé et des services sociaux de la Baie-James. Aucun programme spécifique, d’actions ou de budgets pour le secteur de la santé mentale dans un contexte de changements climatiques n’a été repéré. Discussion et conclusion : Les résultats obtenus de l’examen de la portée (scoping review) ont permis de dégager quatre recommandations adressées aux décideurs pour mieux prendre en considération les risques induits par les changements climatiques. Ces recommandations sont : 1) œuvrer à accroître la recherche sur les liens entre les changements climatiques et la santé mentale ; 2) développer et utiliser un cadre commun en gestion des risques ; 3) Améliorer la communication des risques auprès de la population ; 4) Intégrer l’étape du bilan dans le processus de gestion des risques. L’examen des rapports annuels a permis de constater que les établissements se concentrent davantage sur la disponibilité et l’accès aux infrastructures plutôt que le volet de la santé mentale ou de la santé en général. Un positionnement des enjeux aux changements climatiques par le MSSS dans leur planification stratégique pourrait être favorable aux projets ou aux initiatives en ce sens. Les résultats de cette recherche permettent de statuer que le développement conceptuel d’un cadre de gestion intégrée des risques en mode projet permettrait de mieux prendre en considération les risques induits par les changements climatiques sur la santé mentale au Québec. -- Mot(s) clé(s) en français : changements climatiques, aléas climatiques, événements météorologiques extrêmes, gestion des risques, système de santé, établissements de santé, santé mentale. -- ABSTRACT : Background : Natural disasters due to climate change are more common because of the increase in greenhouse gases. Their impacts are numerous on the health system and they pose a significant ongoing and long-term challenge for the health and well-being of the populations involved. The objective of this thesis is to examine the extent to which it could be beneficial to define and implement an integrated risk management framework to better take into consideration the risks induced by climate change on the mental health of populations in Quebec. Methodology : Between 2010 and 2020, before the COVID-19 pandemic, a scoping review was carried out to explore how risk management is approached internationally in health and social services. Afterward, the annual management and financial reports of health establishments in Quebec were examined. The goal was to inspect if and how this risk management figures among the priorities of different health establishments in Quebec before the COVID-19 pandemic between 2015 and 2020. Result : A total of 15 articles from the OECD countries were analyzed in the scoping review. The majority of these articles were review papers (systematic review or scoping review) or perspective articles (opinion articles) that have been peer-reviewed. No articles studied how the management of risks induced by climate change on mental health was approached by other health and social services internationally. These articles address the following themes ; a framework to manage the risks related to climate change ; an appreciation of the population’s experience when confronted by natural disasters ; and an exploration the risks, the vulnerabilities, the impacts and the determinants of health. Nevertheless, these results remain interesting given they can orient, guide and support decision-makers in managing more efficiently the mental health risks. Furthermore, they allow the identification of incomplete processes or actions that health establishments must strengthen to increase their resiliency to climate change. The 325 annual management reports as well as the financial reports of the CISSS, CIUSSS, CHU, Quebec institutes and the MSSS before the COVID-19 pandemic were analyzed, between the period of 2015 to 2020. Twenty-six mentions of a specific budget for floods in the financial reports of health establishments have been identified. Among these, three mentions are made for a psychosocial component to civil security for floods. We found two mentions linked to a budget on climate change in the financial reports of two CISSS. However, the financial reports do not elaborate on the details of these budgets, and it is difficult to trace the more elaborate information in the annual management reports. Sixteen mentions of climate change come from 9% (3/33) of health establishments. They are the MSSS, the CISSS de la Côte-Nord and the Cree Council of Health and Social Services of James Bay. No specific program, actions or budgets for the mental health sector in the context of climate change have been explicitly identified. Discussion and conclusion : The results obtained from the scoping review made it possible to identify four recommendations for the decision-makers to better address the risks induced by climate change. These recommendations are ; 1) to increase research on the links between climate change and mental health issues ; 2) to develop and use a common risk management framework ; 3) to improve risk communication with the population ; 4) to include the post-project review step into the risk management process. A review of the annual reports revealed that establishments focus more on the availability and accessibility to infrastructures rather than mental health or general health. Integrating climate change issues in the MSSS strategic planning could be favourable to future projects or initiatives.
