Abstract
En 2007, dans la foulée des modifications réglementaires liées à la « Loi 90 », le Québec se dotait d'un modèle provincial d'ordonnance collective de contraception hormonale (OC de CH) permettant aux infirmières et aux pharmaciens communautaires d'initier la contraception hormonale à des femmes en bonne santé pour une période maximale de 6 mois, sans consultation médicale préalable (« task-shifting »).Afin d'évaluer l'implantation de cette nouvelle stratégie de prévention des grossesses non planifiées, une enquête a été menée auprès des infirmières et étudiantes en soins infirmiers ayant suivi la formation en contraception hormonale (traditionnelle ou en ligne), élaborée par l'Institut national de santé publique du Québec (INSPQ) et l'Ordre des infirmières et infirmiers du Québec (OIIQ), et occupant un emploi comme infirmières dans le système de santé québécois.L'objectif était de mieux comprendre cette nouvelle pratique infirmière et de déterminer les facteurs qui influençaient :• son adoption;• son intensité;• sa rapidité d'adoption après la formation.L'initiation de la contraception hormonale par les infirmières dans le cadre d'une OC de CH étant considérée comme une innovation, le questionnaire utilisé dans cette étude a été développé à partir de l'approche de la diffusion des innovations d'Everett M. Rogers.Plus de 3 000 infirmières ont été sollicitées par voie électronique ou postale.Malgré un taux de participation modeste (26,6 %; n = 725), on constate que la pratique infirmière en contraception hormonale est assez répandue puisque :• 57 % des infirmières qui ont suivi la formation font cette pratique;• parmi celles qui font la pratique, 58 % la font dans l'année qui suit la formation et;• 28 % initient la contraception hormonale à 2-5 femmes par mois tandis que près de 40 % l'initient à 5 femmes ou plus par mois.L'adoption d'une OC de CH dans une organisation est le déterminant le plus important de l'adoption de la pratique de l'initiation de la contraception hormonale par les infirmières.Le fait de travailler en clinique jeunesse, dans un CSSS où l'OC de CH est adoptée, d'occuper un poste permanent à temps plein et d'avoir suivi la formation en contraception hormonale dans les premières années de sa disponibilité sont des facteurs importants de l'adoption de cette nouvelle pratique.
