Meconium aspiration syndrome is a major 
\ncontributor to neonatal respiratory distress in infants and 
\nit has been sporadically recognized in neonatal animals. 
\nThis investigation was designed to study the short and 
\nlong term effects of meconium and amniotic fluid in the 
\nlungs of neonatal rats. Seven-day-old rats (n=123) 
\ndivided in three groups were intratracheally inoculated 
\nwith saline solution, amniotic fluid or meconium. Rats 
\nwere euthanatized on 1, 3, 7, 14, 28, 56 and 112 
\npostinoculation days (PID) and the lungs were examined 
\nby light microscopy. Saline solution did not induce any 
\nchange while amniotic fluid elicited only a mild foreign 
\nbody response which disappeared by PID 14. In contrast, 
\nmeconium induced an exudative alveolitis characterized 
\nby recruitment of neutrophilsn in the bronchoalveolar 
\nspaces. Meconium also induced atelectasis, 
\nhyperinflation and thickening of alveolar septa all of 
\nwhich had disappeared by PID 14. Starting at PID 7, 
\nneutrophils were progressively replaced by 
\nmacrophages, giant cells, and some fibroblasts. There 
\nwere sporadic foci of mineralization starting at PID 14 
\nand lasting up to PID 112. Some mineralized foci 
\nbecame lined with cuboidal epithelial cells at PID 28. 
\nMeconium was slowly degraded but still evident by PID 
\n112. It was concluded that inoculation of meconium in 
\nneonatal rats induces acute microscopic changes typical 
\nof meconium aspiration syndrome. The long term lesions 
\ninduced by meconium consisted of persistent multifocal 
\nhistiocytic alveolitis and bronchiolitis reaction with 
\noccasional foci of calcification.
