Bakgrunnur: Munnslímhúðarbólga er ein erfiðasta aukaverkun sem sjúklingar í geislameðferð á höfuð- og hálssvæði fá. Til að greina breytingar í munnholi og skipuleggja viðeigandi íhlutun er mikilvægt að nota réttmætt og áreiðanlegt matstæki. Megin tilgangur þessarar rannsóknar var að meta próffræðilega eiginleika íslenskrar útgáfu Oral Assessment Guide (OAG) matstækisins.
\nAðferð: Rannsóknin var framsýn með endurteknum mælingum hjá samfelldu úrtaki sjúklinga ≥18 ára sem fengu geislameðferð á höfuð- og hálssvæði á geislameðferðardeild Landspítala frá maí 2017 til ágúst 2018. Munnhol var metið af tveimur heilbrigðisstarfsmönnum á níu tímapunktum, fyrir meðferð (T0), á meðan á meðferð stóð (T1-T7) og tveimur vikum eftir að meðferð lauk (T8) með Oral Assessment Guide (OAG) matstækinu (stig 8–24) og World Health Organization Oral Toxicity Scale (WHO-OTS) (gráða 0–4). Breytingar í húð voru metnar með Radiation Therapy Oncology Group (RTOG) matstækinu. Vanlíðan var metin með Mat á vanlíðan (Distress thermometer-DT) skimunar-tækinu og einkenni með Edmonton Symptom Assessment System revised -ESASr einkennamats-tækinu. Næringarástand var metið samkvæmt klínískum leiðbeiningum Landspítala um Mat á næringarástandi. Gögn voru greind með lýsandi og ályktunartölfræði til þess að meta áreiðanleika milli matsmanna og hugsmíðaréttmæti. ROC -greining var gerð til að greina viðmiðunargildi á OAG fyrir alvarlega munnslímhúðarbólgu. Gögn voru skráð og vistuð í töflureikni Microsoft Excel og úrvinnsla gerð í tölfræðiforritinu SPSS.
\nNiðurstöður: Alls uppfyllti 21 sjúklingur af 28 skilyrði og þáði þátttöku, 13 (62%) karlar og 8 (38%) konur. Meðalaldur var 58,5 ár (sf=12,8 ár, spönn 38-80 ár). Flestir þátttakanda voru með krabbamein í munnholi eða 16 (76%), 13 (62%) fengu geislameðferð og 8 (28%) fengu samhliða geisla- og krabba¬meinslyfjameðferð. Á meðferðartímabilinu jukust meðaltalsskor um einkenni munnslímhúðarbólgu bæði samkvæmt OAG og WHO-OTS. Fylgni á milli skora matsmanna fyrir OAG var sterk og marktæk á öllum tímapunktum og fylgni á milli skora matsmanna á WHO-OTS var mishá eftir tímum. Fylgni á milli OAG og WHO-OTS var veik til miðlungssterk á tímabilinu og var mest á T7 (r=0,649, p<0,05 ). Styrkur vanlíðanar og styrkur allra einkenna á ESASr jókst yfir meðferðartímabilið. Á T5 var marktæk fylgni á milli munnslímhúðarbólgu og vanlíðanar (r=0,543, p <0,05) og á milli munnslímhúðarbólgu og verkja (0,639, p <0,01), lystarleysis (r=0,490, p <0,05) og þreytu (r=0,509, p <0,05) á ESASr. Veik til miðlungssterk fylgni var á milli munnslímhúðarbólgu og næringarástands á öllum tímapunktum og sterkust á T0 (r=0,525 p <0,01). ROC greining sýndi viðmiðunargildið 14 á OAG með besta næmi og sértæki til að greina alvarlega munnslímhúðarbólgu í samanburði við WHO-OTS. Alvarleg munnslím¬húðarbólga (OAG ≥14 stig) kom fram hjá fjórðungi sjúklinga í annarri viku meðferðar og var til staðar hjá tæpum helmingi sjúklinga tveimur vikum eftir að meðferð lauk.
\nÁlyktun: Niðurstöður þessarar rannsóknar takmarkast af litlu úrtaki en benda til þess að íslensk útgáfa að OAG sé áreiðanleg og réttmæt en að starfsfólk þurfi þjálfun og eftirfylgd í að nota matstækið.

\nBackground: Oral mucositis is a common and difficult side-effect of radiation therapy to the head and neck area. To detect changes in the oral cavity and plan appropriate intervention, a valid and reliable tool must be used. The main aim of this study was to test the psychometric properties of the Icelandic version of the Oral Assessment Guide (OAG).
\nMethod: This was a descriptive prospective study with repeated measurements in a continuous sample of patients ≥18 years of age who received treatment with radiation to the head and neck area at Landspitali- The National University Hospital of Iceland from May 2017 to August 2018. The oral cavity was assessed by two healthcare professionals at nine time points, before treatment (T0), during treatment (T1-T7) and two weeks after the end of treatment (T8) with the Oral Assessment Guide (OAG) (score 8–24) and the World Health Organization Oral Toxicity Scale (WHO-OTS) (grades 0–4). Skin changes were assessed with the Radiation Therapy Oncology Group (RTOG) tool. Patients were asked to self-assess distress with the Distress thermometer and symptoms with the Edmonton Symptom Assessment System Revised -ESASr. Nutritional status was assessed according to hospital clinical guidelines. ROC analysis was performed to detect the cut-off point of OAG for severe oral mucositis. Data was saved and stored with Microsoft Excel and analyzed by using SPSS Statistics for Windows.
\nResults: Twenty-one of 28 patients met the inclusion criteria and accepted participation, 13 (62%) men and 8 (38%) women. The mean age was 58,5 years (sf=12,8, range 38-80 years). The majority had oral cancer 16 (76%), 13 (62%) received radiotherapy and 8 (28%) received concurrent radio- and chemotherapy. During the treatment period, the mean severity score of oral mucositis symptoms increased according to both the OAG and WHO-OTS. The inter-rater correlation for OAG was strong and significant and the inter-rater correlation for WHO-OTS varied over time. A weak to moderate correlation was found between OAG and WHO-OTS, strongest at T7 (r = 0.649, p <0.05). The severity score of distress of all ESASr symptoms increased during the treatment period. The correlation between distress and oral mucositis was significant at T5 (r=0.543, p<0.05). The correlation between oral mucositis and the severity of ESASr symptoms was significant at T5 for pain (0.639, p<0.01), loss of appetite (r=0.490, p<0.05) and fatigue (r=0.509, p<0.05). There was a weak to moderate correlation between oral mucositis and nutritional status at all time points, strongest at T0 (r=0.525, p<0.01). ROC analysis showed the best sensitivity and specificity for the cut-off value of 14 for OAG to diagnose severe oral mucositis compared to the WHO-OTS. Severe oral mucositis (OAG ≥14 points) was diagnosed in one fourth of the patients in the second week of treatment and in half of the patients two weeks after the end of radiotherapy.
\nConclusion: The results of the study are limited by a small sample but indicate that the Icelandic version of the OAG is valid and reliable. The staff however needs further training and follow-up to use the OAG assessment tool.
