Neurocognitive, anatomical and functional markers of adolescent suicidal behaviors: a multimodal investigation in clinically depressed adolescents

Abstract
Background: Suicidal behaviors are a major source of mortality and morbidity among adolescents.Given maturational changes that occur in cortical and subcortical structures during adolescence, we tested whether atypical brain structural measures are associated with a suicide attempt history.Methods: Three groups of adolescents (N=92; 79% females, mean age=15.9years, range=11.6-18.1)were assessed: patients with a depressive disorder and a history of suicide attempt (SA, n=28); psychiatric controls (PC, n=34) with a depressive disorder but without a history of suicide attempt; and healthy controls (HC, n=30).Regional cortical thickness and surface area, and subcortical volumes were derived from T1-weighted anatomical MRI scans acquired at 3T.Results: Significant group differences in surface area were found in prefrontal, temporal, and parietal regions, as well as in the volume of several subcortical nuclei (p-FDR corrected<0.05),but not in cortical thickness.Post-hoc analyses indicated that morphological alterations primarily differentiated SA from HC but not from PC.However, SA exhibited positive correlations between age and cortical thickness in temporal cortices and right insula, and between age and right putamen volume (i.e.thicker regional cortex and larger subcortical volumes with age); in contrast, these correlations were negative in both PC and HC (i.e.thinner regional cortex and smaller subcortical volumes). Limitations: Sample sizes, cross-sectional findings and psychiatric heterogeneity.Conclusion: Macroscopic structural differences in several brain regions differentiated adolescents with a history of suicide attempt from healthy controls, but not from adolescents with comparable depressive disorders.However, depressed adolescents who attempted suicide may present atypical
