La prise en charge des entorses de cheville aux urgences est le motif le plus fréquent de consultation. L'utilisation d'une règle de décision clinique devient donc pertinente, d'autant que le bilan radiographique est le plus souvent normal. L'analyse des travaux de l'équipe du Dr. STIELL ayant réalisé la règle de décision clinique d'Ottawa est intéressante confrontée à son applicabilité : - Un examen clinique rigoureux et comparatif permet une réduction de clichés sans omettre de lésions osseuses. - Les critères sont cliniquement simples et accessibles, compréhensibles et peu nombreux. - La plupart des études exposées dans cette thèse montrent la parfaite adéquation de ces critères pour la prise en charge d'un traumatisme de la cheville dans un service d'accueil d'urgence. - Il est important pour obtenir de bons résultats que ces règles soient appliquées de façon stricte et sans écart par une équipe bien formée. -Malgré ses avantages, cette règle clinique est relativement peu utilisée. Un algorithme est donc proposé pour son introduction dans un service d'urgence.
