Una distrofia muscular prevalente poco diagnosticada: serie de casos clínicos de distrofia miotónica tipo 1 (dm1)

Abstract
ANTECEDENTES: La Distrofia Miotónica Tipo 1 (DM1), es la distrofia muscular más frecuente en adultos, esta serie de casos clínicos es la segunda conocida en Colombia, la primera realizada en 1983 por Pradilla. METODOLOGÍA: La informado se obtuvo de voluntarios con DM1 por entrevista directa y examen físico; se aplicaron, el Montreal-Cognitive Assessment (MoCA test), Fatigue Severity Scale (FSS), la Escala de Somnolencia Epworthel (ESS), el Inventario de depresión de Beck (BDI); la severidad del compromiso muscular se evaluó según la Muscular Impairment Rating Scale (MIRS); se realizaron análisis de frecuencias; se buscaron correlaciones usando el coeficiente de correlación de Spearman, la prueba Chi-cuadrado y el test exacto de Fisher. RESULTADOS: Treinta y siete pacientes fueron seleccionados con diagnóstico clínico, electrofisiológico y/o genético de DM1, siendo los pilares diagnósticos la debilidad muscular y las descargas miotónicas. Entre las principales complicaciones sistémicas se encontraron: alteraciones cardiacas (53,8%), respiratorias (66,6%), endocrinológicas (38,5%), digestivas (59,5%), psiquiátricas (27%) y cognitivas (29,7%). Los síntomas más frecuentes fueron: somnolencia excesiva diurna (78,4%) y fatiga (81,1%). Los pacientes masculinos presentaron un fenotipo más severo tanto en manifestaciones motoras como no motoras de la enfermedad. CONCLUSIÓN: Esta serie de casos demuestra la prevalencia de las principales características clínicas de la DM1 y las correlaciones entre ellas; también provee la base de datos necesaria para desarrollar estudios prospectivos, con modificadores sintomáticos y preventivos. Se debe ofrecer un manejo multidisciplinario a las personas con DM1 siendo el primer paso, conocer las diferentes formas de expresión clínica, priorizando controles médicos periódicos para la prevención y detección temprana de complicaciones multisistémicas, mejorando así la calidad y la esperanza de vida de estos pacientes.
