La prise en charge nutritionnelle des prématurés joue un rôle primordial dans leur croissance et leur développement. L objectif est d atteindre une croissance se rapprochant de celle du fœtus normal de même âge gestationnel. Objectifs : Comparer la prise en charge nutritionnelle de prématurés de moins de 28 SA hospitalisés dans deux services de Néonatologie et évaluer leur croissance à 36 semaines d âge post-conceptionnel. Méthodes : Etude rétrospective évaluant la croissance et la prise en charge nutritionnelle de 40 prématurés de moins de 28 semaines d aménorrhée hospitalisés dans le service de Néonatologie du CHU Grenoble et dans celui du CHU Québec. Résultats : Les groupes sont comparables en termes d âge moyen, de sexe et pour les données morphométriques à la naissance. La durée d intubation est significativement plus longue pour Québec sans différence significative concernant le taux de dysplasie broncho-pulmonaire, et un taux de sepsis secondaire plus important pour Québec. Il n y a pas de différence en ce qui concerne le taux de retard de croissance extra-utérin à 36 semaines d âge post-conceptionnel (80% pour Grenoble vs 65% pour Québec ; p = 0,48), les poids moyens se trouvant au 6ème percentile pour Grenoble et au 10ème percentile pour Québec. La durée d alimentation parentérale ne diffère pas entre les deux groupes (34 jours vs 36 jours ; p = 0,75). L alimentation entérale est débutée de façon plus précoce pour le groupe Grenoble. Conclusion : La prise en charge nutritionnelle ne paraît pas optimale malgré des apports proches des recommandations. Des analyses complémentaires sont nécessaires pour étudier le devenir de ces enfants à plus long terme.
