Declínio Cognitivo e Depressão nos idosos do Concelho de Coimbra

Abstract
Argumento: diversas investigações referem que os sintomas depressivos são uma dificuldade frequente para a população geriátrica que está associado ao declínio cognitivo. Actualmente em Portugal ainda se sabe pouco acerca dos factores que contribuem para o declínio cognitivo. Objectivos: Pretendemos verificar o papel das variáveis sociodemográficas e dos sintomas depressivos no declínio cognitivo. Metodologia: A amostra é representada por 356 idosos, dos quais 24,4% são homens e 75,6% são mulheres. Esta amostra foi dividida em dois grupos: idosos sem declínio (n = 172), e idosos com declínio (n = 184). Na recolha de dados utilizámos o Montreal Cognitive Assessment (MoCA) para avaliar o grau de declínio dos idosos e aplicámos o Geriatric Depression Scale (GDS) para avaliar a intensidade dos sintomas depressivos. Resultados: A média do declínio cognitivo é inferior às pontuações de referência, ao passo que a média dos sintomas depressivos era superior às pontuações de referência. O declínio cognitivo é maioritariamente afectado pela idade (> de 81 anos) e escolaridade (analfabetos), tendo os idosos mais velhos resultados inferiores, ou seja, quanto maior a idade maior o declínio. O género e o estado civil demonstram igualmente algum declínio cognitivo, embora não tão evidenciado. Os idosos com declínio cognitivo grave apresentam maior índice de sintomas depressivos. As variáveis sociodemográficas demonstraram ter influência no declínio cognitivo. Os idosos mais novos, aparentemente são menos vulneráveis ao declínio cognitivo quando têm sintomas depressivos. Conclusão: Os idosos desta amostra têm, na sua grande maioria declínio cognitivo e sintomas depressivos que podem ser influenciados pela institucionalização, isolamento familiar, entre outras. Estes resultados sublinham a necessidade de estratégias de prevenção e de intervenção, quer para o declínio cognitivo, quer para a sintomatologia depressiva. / Argument: Many investigations say that depressive symptoms are a common difficulty to the geriatric population and i tis related to cognitive decline. At this moment in Portugal, not much is known about the factors that contribute to cognitive decline. Objectives: we want to verify the way in which sociodemographical variables work in cognitive decline. Methodology: The sample is built by 356 elders, from which 24,4% are men and 75,6% are women. This sample has been divided in two groups: elders without decline (n = 172), and elders with decline (n = 184). In data getting, we used Montreal Cognitive Assessment (MoCA) to evaluate the degree of elderly decline and we applied Geriatric Depression Scale (GDS) to evaluate the intensity of depression symptoms. Results: The mean of cognitive decline is smaller than the referenced scores whereas the mean of depressive symptoms was greater than the referenced scores. Cognitive decline is greatly affected by age (> 81 years) and education (illiterate), having lower scores, which means the greater the age, the greater the decline. The gender and civil state are also commented to cognitive decline, but they aren’t that clear. The elders with serious cognitive decline have a greater depressive symptoms indeed. Sociodemogaphical variables have been noted as having influence in cognitive decline. Youngers elders are apparently less vulnerable to cognitive decline when they have depressive symptoms. Conclusion: The elders from this sample have in majority cognitive decline and depressive symptoms, that can be influenced by institutionalization, being away from the family and others. These results underscore the need for prevention strategies and intervention, both for cognitive decline, both for depressive symptoms.
