Introducción: La realidad virtual es una simulación generada por computadora de un entorno o ambiente real, en el que se genera una interacción altamente motivadora del sujeto con el entorno virtual, a través de una interfaz hombre-máquina. Objetivo: Analizar los efectos de la realidad virtual en miembro superior sobre la calidad de vida y marcadores de fisioterapia en pacientes que han sufrido un accidente cerebrovascular. Metodología: Siguiendo las directrices de los Elementos de Información Preferidos para Revisiones Sistemáticas y Metaanálisis (PRISMA), se revisaron sistemáticamente estudios de Medline (PubMed), Physiotherapy Evidence Database (PEDro) y Scopus desde el inicio de las bases de datos hasta el 5 de mayo de 2024. Con el fin de evaluar la eficacia de la rehabilitación de la extremidad superior con realidad virtual en comparación con el tratamiento convencional en pacientes que habían sufrido un ictus, sobre marcadores fisioterápicos y de calidad de vida. Se utilizó la escala PEDro y McMaster para evaluar la calidad metodológica y la herramienta de Cochrane para evaluar el riesgo de sesgo. Resultados: Entre los 208 ensayos identificados en la búsqueda, solo 8 cumplieron con los criterios de inclusión y exclusión. En las escalas de calidad metodológica obtuvieron resultados que se correspondían con calidades buena, muy buena o excelente. Los resultados se dividieron en calidad de vida y marcadores de fisioterapia. Se observaron cambios en la fugl meyer assessment scale (FMA), box and block test (BBT), Functional Independence Measure (FIM), Montreal Cognitive Assessment (MoCA), Jebsen–Taylor hand function test (JTT), National Institutes of Health Stroke Scale (NIHSS), Activities of Daily Living/ Instrumental Activities of Daily Living (ADLs/IADLs), range of movement (ROM), la velocidad y el tiempo de ejecución de los movimientos. Siendo significativo (p < 0,05) principalmente en la escala FMA, BBT, ROM, en la velocidad y en el tiempo de ejecución. Conclusiones: El uso de realidad virtual en la rehabilitación de pacientes con ictus, es más efectiva que la rehabilitación convencional aislada para marcadores de fisioterapia y de calidad de vida.
