Abstract
LE rapport bénéfice-risque de la greffe d îlots reste mal évalué.Le but de cette étude est de déterminer l évolution des complications macroangiopathiques 5 ans après greffe d îlots.21 des 36 patients greffés d îlots suivis prospectivement à Lille,qui ont une durée de suivi d au moins 5 ans ont été inclus.Leurs caractéristiques initiales étaient un âge de 43+/-7ans,une durée de diabète de 28+/-9 ans,un BMI de 24+/-2kg/m2;8 patients sur 21 ont reçu une greffe rénale 23+/-10 mois auparavant;immunosuppression:Edmonton.Ces 21 patients ont bénéficié avant la greffe puis annuellemet d un écho-doppler carotidien,des membres inférieurs,d un enregistrement ambulatoire de la pression artérielle et d un dépistage de l ischémie myocardique silencieuse.Une coronarographie est réalisée si ce dépistage est positif.3 patients îlots seuls et 2 îlots après rein ont perdu la fonction c-peptidique à 5 ans.9 patients étaient insulino-indépendants,8/9 avec une hémoglobine glyquée 1 ou 1 traitement antihypertenseur:38%versus 75%).3 patients dont 2 îlots après rein avaient des complications macroangiopathiques avant la greffe (2 pontages des membres inférieurs dont 1 des patient avait une amputation un stent coronaire).Il n y a pas eu d évènement cardiovasculaire aigu durant ces 5 ans.Le dépistage de la macroangiopathie était anormal avant versus après pour l écho-doppler carotidien:37%versus 52%;des membres inférieurs 36%versus 85%(22versus25% si la médiacalcose était exclue),calcifications :14% versus 70%,épaisseur intima-média (0.676+/-0.16 versus 0.672+/-0.126mm),le test non invasif de dépistage de l ischémie myocardique silencieuse:19% (avec 3 coronarographies normales en dehors d une calcification)versus24%( dont4 stents).La greffe d îlots n est associée à aucun évènement cardiovasculaire aigu chez ces 21 patients avec une longue durée du diabète,dont 8 patients greffés de rein,au prix d un dépistage et d un traitement systématiques des complications macroangiopathiques.
