Evaluation de la prise en charge du risque rabique aux centres hospitaliers d Annecy et de Grenoble chez les patients mordus en 2006

Abstract
Les indications sérovaccinales contre la rage publiées par l Organisation Mondiale de la Santé (OMS), inchangées depuis 1996, semblent à ce jour inapplicables compte tenu de l épizootiologie française. L analyse rétrospective de 476 dossiers de patients ayant consulté pour morsure aux Urgences ou aux Centres Anti-Rabiques (CAR) de Grenoble (CHU) et d Annecy (CHR) sur l année 2006 a permis de montrer que la prise en charge était globalement cohérente avec le référentiel des CAR, mais très éloignée des recommandations de l OMS (insuffisance de sérovaccinations). Alors que les cas pris en charge sont les mêmes, le CAR d Annecy privilégie la vaccination seule, alors que celui de Grenoble adopte une surveillance vétérinaire de l animal mordeur. La sérothérapie est pratiquée dans moins de 3% des cas. Aucun patient en contact avec une chauve-souris n a reçu d immunoglobulines. La principale incohérence concerne les patients mordus en France par un chien ou chat fuyard : le patient est le plus souvent vacciné, sans sérothérapie, et parfois non traité. La surveillance vétérinaire est inopérante (effective dans moins de 25% des cas), et n a entraîné de modification de l attitude thérapeutique que dans un seul cas (arrêt de vaccination) ; aucun patient n a été rappelé pour débuter un traitement. Cette attitude inhomogène dans une même région est consécutive à la carence de recommandations adaptées à notre pays. C est pourquoi nous avons finalement tenté de proposer un référentiel, affranchi de surveillance vétérinaire, et dont la seule alternative à l abstention en cas de contact de catégorie trois, serait la sérothérapie.
