L'intestin est reconnu comme un organe métabolique clé impliqué dans le développement des complications de l'obésité et du diabète. En présence d'obésité, l'intestin grêle développe un état local de résistance à l'insuline, un métabolisme lipidique altéré et une inflammation ainsi qu'une perméabilité paracellulaire accrue. En raison de l'environnement complexe des cellules épithéliales intestinales, il n'est pas clair à ce jour si la dyshoméostasie intestinale peut être induite par des altérations endogènes liées à l'obésité de l'hôte. L'objectif de cette étude était d'évaluer l'impact individuel de l'hyperglycémie, la dyslipidémie et l'inflammation sur les fonctions épithéliales intestinales en utilisant des organoïdes de crypte intestinale. Les organoïdes, dérivés du duodénum de souris mâle C57BL/6, ont été incubés 24 h dans des conditions inflammatoires, dyslipidémiques avec un taux élevé d'acide gras libres et hyperglycémiques. L'expression génique ainsi que la sensibilité à l'insuline ont été mesuré par qPCR et analyse densitométrique, respectivement. Nous montrons que les organoïdes, et donc les cellules de l'épithélium intestinal, répondent bien à l'insuline car cette hormone induit une activation dose-réponse de la phosphorylation d'Akt, un effecteur clé de cette voie de signalisation. La signalisation de l'insuline a été réduite par le cocktail inflammatoire et par la dyslipidémie, alors que l'hyperglycémie réduit la sensibilité à l'insuline à des niveaux élevés de glucose. L'état inflammatoire affecte l'expression des gènes liés à la perméabilité paracellulaire intestinale, à la réponse immunitaire et inflammatoire, au métabolisme des lipides et des lipoprotéines et au transport des nutriments, alors que la dyslipidémie a eu peu d'effet sur ces gènes. Cette étude suggère que les changements endogènes de l'hôte liés à l'obésité représentent un mécanisme important dans le développement des altérations de l'intestin grêle lors du développement de l'obésité. Ces altérations auraient à leur tour un impact sur les complications systémiques, notamment en augmentant le taux de lipoprotéines et de glucose libéré en circulation.
