Introduction : la maîtrise de l'information se situe au carrefour de compétences liées à la communication, à l'information et aux technologies. La médecine est particulièrement exposée aux enjeux d'une telle maîtrise, par sa démarche qualitative de pratique fondée sur des éléments probants (EBM). Dans cette thèse, je me suis intéressé aux internes en D.E.S. de médecine générale à Nantes, sous les différents angles de leur formation initiale, de leurs compétences informationnelles médicales et de leurs besoins de formation. Méthodes : un inventaire des formations initiales disponibles à Nantes a été dressé, accompagné d'exemples d'auto-formation en ligne, pour ce niveau universitaire. Deux enquêtes prospectives d'évaluation quantitative ont permis d'explorer les compétences informationnelles et besoins de formation des étudiants en 1ère et en 3e année de ce D.E.S. Les résultats obtenus ont été comparés avec le référentiel normatif de l'Université du Québec (PDCI). Résultats : si les formations disponibles à Nantes couvrent l'intégralité du référentiel choisi, les compétences des étudiants sont quant à elles largement lacunaires. Ceux-ci sont globalement mal renseignés sur l'offre de formation, en particulier locale ; leurs pratiques informationnelles médicales sont souvent rudimentaires, tant par les ressources et outils utilisés que par la qualité de leur exploitation. La prise de conscience de ces lacunes semble partielle, puisque les souhaits de formation négligent notamment les compétences critiques et la réalité de l'autonomie d'information du patient. Discussion : la confusion collective entre compétences technologiques et informationnelles retarde la mise en place d'une formation de qualité tenant compte de l'évolution des pratiques informationnelles des étudiants. Cette formation serait idéalement collaborative et mutualisée, suivant un référentiel adapté à ce D.E.S. de médecine générale.
