Introducción: Las enfermedades crónicas no transmisibles son un problema de salud pública, destacando las alteraciones del metabolismo de la glucosa. Se sabe que el uso de anticonceptivos orales (ACOs) puede alterar dicho metabolismo; sin embargo, muchos estudios no son concluyentes. El objetivo fue evaluar si las mujeres en edad fértil que usan ACOs comparadas con su no uso, tienen diferencias en los niveles séricos de glucosa, insulina y/o HOMA-IR. Métodos: Revisión sistemática siguiendo la guía Cochrane que incluyó estudios que respondían a la pregunta PECOS. Se usaron las bases MEDLINE, LILACS, Cochrane, Scopus, y Web of Science. La evaluación de riesgo de sesgos se hizo mediante las escalas “New Casttle Ottawa” y “JBI Critical Appraisal Checklist for Quasi-Experimental Studies”. Se realizaron metaanálisis usando modelos aleatorios según el método de Dersimonian-Laird. Resultados: Se encontraron 2626 artículos, de los cuales fueron incluidos 63. Todos los estudios tuvieron bajo riesgo de sesgo. En mujeres con síndrome de ovario poliquístico (SOP), hubo disminución marcada del HOMA-IR: -0.45 (IC 95% -0.74; -0.15) luego de usar ACOs. En mujeres sin SOP, los niveles de insulina en ayunas aumentaron en promedio 5.57 pmol/L (IC 95% 2.25; 8.89), y el HOMA-IR subió en 0.26 (IC 95% 0.02; 0.50). No hubo variación en los valores de glucosa para los dos grupos. Los metaanálisis tuvieron heterogeneidad moderada/alta. Conclusión: En mujeres sin SOP, los niveles de insulina aumentaron luego de usar ACOs; mientras que, los niveles de HOMA-IR disminuyeron en mujeres con SOP y aumentaron en mujeres sin SOP. No se detectó efecto sobre la glucosa sérica.
