Són diversos els factors socials que incideixen en les diferents etapes per les quals travessa la malaltia mental; una d'elles és la relacionada amb el canvi ambulatori al centre de salut mental d'adults, on l'adolescent afectat d'un trastorn mental greu s'ha d'adaptar a la nova dinàmica d'atenció i equip de professionals suposant-li en múltiples ocasions una situació estressant que pot arribar a afectar la correcta vinculació i seguiment posterior al nou dispositiu. En el present treball es realitza una recerca bibliogràfica sobre els principals factors socials que determinen aquesta transició entre dispositius de salut mental ambulatòria i que són susceptibles de ser intervinguts des del Treball Social Sanitari per afavorir l'òptima adherència i contribuir a la continuïtat assistencial del centre de salut mental d'adults. L'abordatge dels aspectes socials obliga a l'exercici de comprendre la salut i la malaltia des d'un enfocament biopsicosocial on els tres tipus de factors (biològic, psicològic i social) desenvolupen un paper igual de significatiu en la salut mental dels joves amb patologia psiquiàtrica greu en un moment de canvi com el que emmarca aquesta recerca.
