Erişkin Periferik Arter Hastalarında Kırılganlık İndeksinin Denge ve Düşme Riskine Etkisinin DeğerlendirilmesiÖğrenci: Yusuf BALTACIDanışman: Prof. Dr. S. Ufuk YURDALANAnabilim Dalı: Kardiyopulmoner Rehabilitasyon ZETAmaç: İnsanlarda denge sistemi erken yetişkinlik döneminde optimaldir. Beşinci dekadla periferik arter hastalığı (PAH) ile kırılganlık düzeyi artar; bu bozulma engellilik, düşmeler, hastane yatışı ve hatta ölüm gibi önemli sonuçlara yol açar. Çalışmamız da erişkin PAH tanılı olgularda kırılganlık indeksi skorlarının denge bozukluğu ve düşme riski üzerindeki etkilerinin araştırılması amacı ile planlanmıştır.Gereç ve Yöntem: Araştırmamıza çalışmaya dahil edilme ölçütlerine uyan 35 PAH’lı birey ve 17 PAH dışlanmış birey seçilmiştir. Demografik Verileri alınan katılımcılara; Ayak Bileği Kol İndeksi (ABİ); yüz yüze Edmonton Kırılganlık Ölçeği, Berg Denge Ölçeği (BDÖ) ve Uluslararası Düşme Etkinlik Ölçeği (FES-I) uygulanmıştır.Bulgular: Katılımcıların (n=52) yaş ortalamaları 64,9 ± 6,4 yıl idi. Kırılgan olan grupta hastaların yaşı (n=14 / 60,8y) kırılgan olmayan gruptan (n=38 / 66,4) anlamlı olarak daha yüksek bulundu (p < 0,05). Vaka grubu ve kontrol grubu arasında FES-I skoru, FES-I’e göre düşme riski; Berg denge skoru, Berg denge skoruna göre düşme ve denge riski; Edmonton Kırılganlık Ölçeği’ne göre kırılganlık oranına bakıldığında her iki grupta da yüksek bulundu; iki grup arasında anlamlı (p > 0,05) farklılık görülmedi. Kırılgan olan grupta FES-I skoru ve Berg Denge Ölçeği skoru kırılgan olmayan gruptan anlamlı (p < 0,05) olarak daha düşük bulundu. Kırılgan olan grupta FES-I’e göre düşme riski kırılgan olmayan gruptan anlamlı (p < 0,05) olarak daha yüksek görüldü.Sonuçlar: Çalışmamızda; kırılganlık indeksindeki artışın denge ve düşme riski skorlarını anlamlı olarak etkilediği bulundu. Erişkin PAH tanılı hastaların kırılganlık düzeylerindeki artış, buna bağlı alınacak fizyoterapatik önlemler ve tedavi modellerinin geliştirilmesinin klinik açıdan yararlı olabileceği sonucuna varıldı.
