CONTENU : Il s’agit de proposer un modèle de collaboration domicile/hôpital pour le patient insuffisant cardiaque qui correspond à la fois aux recommandations pour la prise en charge de ces patients et aux interventions les plus pertinentes. Ce modèle souligne le rôle autonome infirmier, respecte et précise le travail du généraliste et s’adapte aux besoins des soignants du domicile. Cette recherche exploratoire concerne un contexte précis : les services de soins de santé primaires de la province de Namur et de Luxembourg qui gravitent autour du CHU Dinant Godinne UCL, site Godinne. METHODOLOGIE : Afin d’élaborer un modèle basé sur les besoins et difficultés des principaux soignants concernés, une méthode qualitative a été choisie. L’échantillon par choix raisonné comprenait des généralistes et des infirmiers du domicile choisis selon des critères précis. Le recueil des données a été réalisé au moyen d’entretiens semi-dirigés individuels, suivant une grille élaborée en référence à la partie théorique. L’analyse des données a été effectuée au moyen d’une matrice catégorielle prédéfinie. RESULTATS : Seize entretiens ont été menés, auprès de huit médecins généralistes et huit infirmiers. Ceux-ci révèlent premièrement l’indisponibilité téléphonique du cardiologue ou d’une personne de référence et le manque d’informations dans la lettre de sortie voire l’absence de courrier pour les infirmiers. Deuxièmement l’éducation du patient insuffisant cardiaque devrait être débutée à l’hôpital par du personnel spécialisé et poursuivie à domicile moyennant un soutien des soignants dans cette tâche. Troisièmement le rôle du généraliste est primordial car cette population de patients présente de multiples pathologies associées. Enfin, les infirmiers du domicile exercent leur rôle propre tant bien que mal vu les faibles informations dont ils disposent. Tenant compte de ces résultats, notre modèle vise une amélioration de l’autogestion, des moyens de communication, un soutien aux décisions cliniques, éléments du Chronic Care Model. Il répond aux recommandations de la Société Européenne de Cardiologie et de NICE.
