Abstract
OBJECTIFS : Evaluer la prise en charge de l'entorse de cheville aux urgences en se référant à la dernière conférence de consensus. Comparer la tolérance et le confort de l'orthèse en résine semi-rigide amovible (RSA) réalisée par notre gypsothérapeute à une orthèse type Aircast®, évaluer la pertinence de lecture des radiographies par les urgentistes. METHODE : Audit clinique selon la méthode de l'HAS. Une analyse rétrospective des dossiers médicaux des patients âgés de 15 à 55 ans consultant les urgences pour une entorse de cheville a été réalisée, à partir des critères suivants : mention des critères d'Ottawa, des conseils de surveillance, prescription de radiographie, du traitement médical, de la kinésithérapie. Une information a ensuite été réalisée auprès des praticiens intervenants aux urgences et des améliorations ont été mises en place : affichage des critères d'Ottawa, ordonnances informatisées conformes aux recommandations (traitement fonctionnel, kinésithérapie, conseils de surveillance). Une analyse prospective a été conduite immédiatement après notre intervention, puis un an après. RESULTAT : Notre étude montre que les pratiques n'étaient pas conformes au consensus. Notre intervention a permis de modifier significativement et durablement la majorité des critères analysés. Seules les prescriptions d'antalgique et de radiiographie n'ont pas évolué. La RSA serait plus confortable et mieux tolérée que l'attelle préfabriquée, sans que nos résultats soient significatifs. Enfin, 10,2 % des interprétations radiographiques sont discordantes entre urgentistes et radiologues. CONCLUSION : Cet audit a permis de faire progresser significativement et durablement nos habitudes. Même si tous les praticiens ne sont pas également accessibles à ce type de travail, nous ne saurions qu'inciter nos camarades urgentistes à réaliser de telles études. Un rappel régulier des bonnes pratiques reste cependant indispensable dans les structures où le personnel change fréquemment.
