Abstract
目的:雄激素受体(androgen receptor,AR)表达在膀胱尿路上皮癌中的临床意义存在争议,通过系统评价分析AR表达与膀胱尿路上皮癌肿瘤特征以及预后之间的关系,并探讨AR能否为膀胱尿路上皮癌预后预测和治疗方案的选择提供依据。<br />方法:利用主题词、关键词、文本词和题名等不同组合,计算机检索中国知网、维普科技期刊、万方、PubMed、Embase、Web of Science数据库和Cochrane图书馆,检索时间均起自建库,截止时间为2018年12月。搜集关于AR和膀胱癌相关的研究。主要评估AR表达与膀胱尿路上皮癌肿瘤组织及癌旁组织、不同分级、分期、大小、数目、有无淋巴结侵袭,以及患者无复发生存和无进展生存之间的关系。系统评价流程依据MOOSE(2010版)对观察性研究推荐的报告规范进行。对检索到的文献进行筛选,Newcastle-Ottawa量表对纳入研究的质量进行评价,对最终纳入研究的信息进行分析总结后利用Revman 5.3和Stata14.0软件进行统计分析,Chi2检验评估纳入研究的异质性,用OR、HR和95%CI评估描述研究结果。<br />结果:共纳入研究24个,来自欧美的8个、亚洲的15个(其中中国的研究10个)和埃及的1个,共包括研究对象3191例,其中膀胱尿路上皮癌2603例,非肿瘤对照588例。对纳入研究进行Meta分析的结果表明,AR在膀胱尿路上皮癌中的表达与肿瘤数目、大小以及性别之间没有关联。AR表达在膀胱尿路上皮癌肿瘤组织中高于癌旁正常组织(OR=5.04;95%CI[1.06-24.01];P=0.04)。AR表达在不同肿瘤分级中的差异与研究中的采用不同WHO膀胱癌分类系统相关,依据1973年WHO分级研究的总体分析结果显示:AR表达在3级肿瘤中高于2级肿瘤(OR=0.54;95%CI[0.35-0.84;P=0.006);依据2004年WHO分级研究的总体分析结果显示:AR表达在低级别肿瘤中高于高级别肿瘤(OR=1.90;95%CI[1.05-3.41;P=0.03)。AR表达在不同分期肿瘤中的差异与种族和采用的结果判定方法相关,在白种人的非肌层浸润性肿瘤中高于肌层浸润性肿瘤(OR=2.31;95%CI[1.16-4.58];P=0.02),采用染色强度判定AR表达时,非肌层浸润性肿瘤中AR表达高于肌层浸润性肿瘤(OR=2.08;95%CI[1.42-3.05];P=0.0002)。AR表达与膀胱尿路上皮癌淋巴结侵袭呈正相关(HR=1.74;95%CI[1.04-2.91];P=0.03)。预后分析结果显示,AR表达与膀胱尿路上皮癌的PFS之间呈负相关(HR=1.25;95%CI[1.06-1.48];P=0.010)。<br />结论:AR在膀胱尿路上皮癌肿瘤组织中的表达高于正常组织,一组明确表达AR的特定患者可能从拮抗AR中获益;AR在肿瘤不同分级、分期中的表达差异与采用不同的WHO分类系统、AR检测结果判定方法和种族有关,与白种人的低分级和低分期肿瘤正相关;AR表达与膀胱尿路上皮癌淋巴结侵袭呈正相关;AR表达与总体膀胱尿路上皮癌患者的无进展生存负相关,AR可以预测该类患者的无进展生存,但相关的结论仍需更多研究进一步证实。
