Tämä Oulun ammattikorkeakoulun opinnäytetyö on toteutettu kuvailevana kirjallisuuskatsauksena. Kirjallisuuskatsaus tarjoaa uusinta tietoa antibioottien käytön vaikutuksesta vankomysiiniresistenttien enterokokki -kantojen (VRE) esiintyvyyteen. Kirjallisuuskatsauksen tarkoituksena oli esittää usean tutkimusartikkelin tarjoama tieto tiiviissä muodossa suomenkielellä terveydenhuollon ammattilaisille. Tavoitteena oli tutkia, miten antibioottien käyttö vaikuttaa VRE-kantojen esiintyvyyteen.

\nVankomysiiniresistentti enterokokki eli VRE on grampositiivinen enterokokkibakteeri, joka on vastustuskykyinen vankomysiini-antibiootille. VRE on moniresistenttisairaalabakteeri eikä sen hoitoon voida käyttää tavallisesti enterokokkien hoitoon soveltuvia antibiootteja. Sairaalapotilaat ovat usein kolonisoituneet VRE:llä, mutta vain harvoilla esiintyy kliinisiä infektioita. Monien mikrobilääkkeiden on todettu edistävän suoliston VRE-kolonisaatiota ja pidentävän sen kestoa.

\nTähän kirjallisuuskatsaukseen etsittiin alkuperäisiä tutkimusartikkeleita viidestä eri viitetietokannasta sisäänotto- ja poissulkukriteereiden, sekä suunniteltujen hakutermien avulla. Tuloksia haettiin vuosilta 2015-2021 suomen ja englannin kielillä. Aineisto analysoitiin luokittelemalla tutkimusten ilmaukset ylä- ja alaluokkiin.

\nTässä kirjallisuuskatsauksessa havaittiin, että karbapeneemien käyttöä pidettiin VRE-tartunnan riskitekijänä, mutta puolestaan atstreonaami pienensi VRE-tartunnan riskiä. Monilogistisessa regressiomallissa havaittiin yhteys metronidatsolin ja VRE-kolonisaation välillä. Korkean paikallisen vankomysiinin käytön havaittiin pidentävän huomattavasti VRE-kolonisaation kestoa verrattuna matalan paikallisen käytön ryhmään, riippumatta yksilötason vankomysiinin käytöstä tai yksikön VRE-tiheydestä. Vankomysiinille, fluorokinoloneille ja meropeneemille pidempiaikainen altistuminen todettiin riskitekijäksi VRE-bakteremialle. Muiden tutkimuksissa esiintyvien antibioottien merkitys VRE-kantojen lisääntymiseen oli joko vähäinen, niillä ei ollut suurta merkitystä tai vähentävistä vaikutuksista ei kerrottu riittävästi alkuperäistutkimuksissa.
