Conception d'un hydrogel macroporeux de chitosane et d'alginate réticulé avec de la génipine pour la capture de cellules cancéreuses de glioblastome

Abstract
RÉSUMÉ: Le glioblastome multiforme (GBM) de stade IV est la forme la plus agressive du cancer du cerveau et touche 45 613 nouveaux patients aux États-Unis par an. Ce cancer se localise dans le cerveau ou dans la moelle épinière et sa gravité réside dans la capacité qu’ont les cellules tumorales de proliférer très rapidement à travers un vaste réseau de vaisseaux sanguins et d’envahir les tissus sains environnants. Malgré les traitements actuels combinant chirurgie et chimiothérapie / radiothérapie, ce cancer reste incurable car il est impossible de détruire la totalité des cellules cancéreuses résiduelles sans endommager les tissus sains du cerveau. Un nouveau traitement, la Gliotrappe, est actuellement en phase de développement entre les équipes de Polytechnique Montréal et de l’Université de Sherbrooke afin de pouvoir détruire les cellules cancéreuses résiduelles. Le principe de ce projet est d’implanter un hydrogel macroporeux qui agit comme trappe à cellules cancéreuses dans la cavité résiduelle après résection de la tumeur. Pour ce faire, un chimio-attractant sera introduit dans le gel afin d'attirer et de retenir les cellules cancéreuses résiduelles. Une fois qu’elles auront migré au sein du gel, une dose de radiothérapie stéréotaxique sera appliquée localement pour les éliminer. Pour satisfaire aux exigences d’une telle application, les hydrogels doivent être biocompatibles, avoir la capacité de laisser entrer les cellules cancéreuses, avoir un module de compression proche de celui des tissus cérébraux, soit entre 1 et 10 kPa, et être stables dans le milieu physiologique. Dans ce projet de maîtrise, une nouvelle formulation à base d’alginate de sodium (SA), de chitosane (CHI) et de génipine (GNP) a été développée pour préparer les hydrogels poreux. L’alginate a été choisi en raison de son caractère non immunogène et de ses propriétés mécaniques similaires à celles des tissus mous du corps humain, et le chitosane pour améliorer l'adhésion cellulaire. Aussi, la combinaison des deux polymères présente l'avantage d'accroître la stabilité des gels lorsqu'ils sont exposés à un environnement ionique, car des liaisons physiques s’établissent entre eux. Cependant, ces liaisons sont réversibles, ce qui implique une diminution progressive de la stabilité des gels lorsqu’ils sont soumis à un environnement ionique. Ainsi, la contribution originale de ce projet de maîtrise est une nouvelle chimie de gel qui augmente leur stabilité, d’une part par l’augmentation de leur concentration en CHI et de l’homogénéité des solutions alginate/chitosane à l’aide de bicarbonate de soude, et d’autre part par la réticulation chimique du chitosane à l’aide de la génipine. ABSTRACT: Stage IV of multiform Glioblastoma (GBM) is the most aggressive form of brain cancer, affecting 45 613 new patients in the United States each year. This cancer can be localized in the brain or spinal cord and is characterized by the ability of tumor cells to easily proliferate through an extensive network of blood vessels and invade the surrounding healthy tissues. Despite current treatments combining surgery and chemotherapy/radiotherapy, this cancer remains incurable because it is impossible to eliminate all residual cancer cells without damaging healthy brain tissue. A new treatment, called the Gliotrap, is currently in development by both teams at Polytechnique Montréal and the University of Sherbrooke to target and kill residual cancer cells. The principle of this project involves implanting a macroporous hydrogel that acts as a cancer cells trap in the residual cavity after tumor resection. To achieve this, a chemoattractant will be introduced into the gel to attract and retain cancer cells. Once the cancer cells are retained into the gel, a dose of stereotactic radiotherapy will be applied locally to eliminate them. To meet the requirements of such an application, the hydrogels must be biocompatible, must accumulate and retain the cancer cells, have a compression modulus similar to brain tissues, ranging from 1 to 10 kPa, and remain stable in the physiological environment. Therefore, in this master's project, a new formulation based on sodium alginate (SA), chitosan (CHI), and genipin (GNP) was developed to prepare porous hydrogels. Alginate was chosen for its non-immunogenicity and mechanical properties similar to human soft body tissues, while chitosan was selected to improve cell adhesion. The combination of these two polymers has the advantage of increasing gel stability in ionic environments, as physical bonds are formed between them. However, these bonds are reversible, which means that gel stability gradually decreases in those environments. The original contribution of this project is a new gel chemistry improving gel stability, by increasing the CHI concentration in the gels and the homogeneity of alginate/chitosan solutions with sodium bicarbonate, as well as by chemically cross-linking chitosan using genipin.
