Bakgrund: Personer med ryggmärgsskada har en minskad styrka i andningsmuskulaturen vilket ökar risken för respiratoriska komplikationer. För att öka styrkan rekommenderas inspiratorisk muskelträning (IMT). Syfte: Syftet med studien var att beskriva kliniska och självskattade effekter av IMT på muskelstyrka i inandningsmuskulatur, andningsfunktion och aktivitetsutförande hos personer med kronisk komplett cervikal eller hög thorakal ryggmärgsskada. Metod: Studien genomfördes som en prospektiv kvasiexperimentell fallstudie med kontrollgrupp där deltagarna i kontrollgruppen inkluderades i interventionsgruppen efter kontrollperioden. Tretton personer genomförde IMT i hemmet under åtta veckor. Utvärdering gjordes med de kliniska utfallsmåtten maximalt inspiratoriskt tryck (MIP), spirometri och 6-minuters push-test (6MPT) samt de patientrapporterade utfallsmåtten Canadian Occupational Performance Measure (COPM) och Patient Global Impression of Change (PGIC). Resultat: Resultatet visade en signifikant förbättring av styrka i inandningsmuskulaturen mätt med MIP, andningsfunktion mätt med forcerad vitalkapacitet (FVC) samt självskattat aktivitetsutförande mätt med COPM. Ingen signifikant skillnad visades för andningsfunktion mätt med forcerad exspiratorisk volym på 1 sek (FEV1) eller Peak Exspiratoriskt flöde (PEF) och inte heller för kondition mätt med 6MPT. Ingen signifikant korrelation visades mellan förändring av MIP och förändring av COPM. Förbättring av generell andningsfunktion enligt PGIC visades hos 69 % av deltagarna. Slutsats: IMT förbättrade styrka i inandningsmuskulatur, andningsfunktion mätt med FVC och självskattat aktivitetsutförande kopplat till andningsfunktion. Resultatet i studien gav ökad kunskap om aktivitetsproblem som patientgruppen identifierade kopplat till nedsatt andningsfunktion. Då deltagarantalet i aktuell studie var litet bör resultaten tolkas med försiktighet.
