INTRODUCTION : Les personnes atteintes de lombalgie chronique et présentant un haut risque de mauvais pronostic sont responsables de la majorité de l'invalidité associée à cette condition. La prise en charge stratifiée pourrait permettre d'augmenter l'efficacité limitée des interventions chez cette population, notamment en proposant de la physiothérapie informée par la psychologie (PIP) au lieu de la physiothérapie usuelle (PU). Pourtant, d'importants problèmes d'implantation lors de la réalisation d'essais cliniques randomisés (ECR) évaluant l'efficacité de ces deux approches de physiothérapie ont été rencontrés. OBJECTIF : Déterminer la faisabilité d'un ECR testant l'efficacité de la PIP comparativement à la PU chez les personnes atteintes de lombalgie chronique présentant un risque élevé de mauvais pronostic selon l'outil de dépistage Start Back et explorer l'efficacité préliminaire des interventions. MÉTHODES : Cette étude de faisabilité évaluait l'efficacité des stratégies de recrutement, l'adhésion au traitement, le risque de contamination et les défis spécifiques. Une formation de deux jours encadrait la PIP. L'intervention comptait 8 rencontres sur 11 semaines. La capacité fonctionnelle, la douleur, la qualité de vie, la kinésiophobie, la catastrophisation, la sensibilisation centrale et l'auto-efficacité ont été évalués à 1, 6, 12 et 24 semaines. RÉSULTATS : Quarante participants ont été recrutés majoritairement par la liste de courriel de l'Université Laval et dix physiothérapeutes ont pris en charge des patients. Le taux de rétention des participants à 24 semaines était de 72,5%. L'adhésion au traitement des participants et des physiothérapeutes était très bonne. Le risque de contamination était faible et les défis spécifiques identifiés, modifiables. Une amélioration significative dans le temps de toutes les variables d'intérêts cliniques, sauf l'auto-efficacité, a été observée sans différence entre les groupes. CONCLUSION : La réalisation d'un ECR évaluant l'efficacité de la PIP et la PU est faisable avec modifications mineures. La PIP et la PU semblent similairement efficaces.
