A orbita e um sitio preferencial de lesoes linfoides que incluem linfomas extranodais e hiperplasias linfoides. A diferenciacao histopatologica entre hiperplasia linfoide e linfoma pode ser dificultada particularmente em biopsias pequenas. Tecido linfoide associado a mucosa (MALT) e um acumulo adquirido de celulas linfoides no estroma conjuntival sempre associado ao epitelio. Os tumores linfoides da conjuntiva devem ser classificados em hiperplasia linfoide ou linfoma dependendo das caracteristicas histopatologicas do infiltrado linfoide. O objetivo deste estudo e de analisar as caracteristicas histopatologicas e imuno-histoquimicas que nos permite o diagnostico correto dos tumores linfoides tanto da orbita quanto da conjuntiva. Os 55 casos de tumores orbitarios e 35 casos de lesoes linfoides conjuntivais foram obtidas do Registro de Patologia Ocular da Universidade McGill em Montreal - Canada e da Universidade Federal de São Paulo. Preparados histopatologicos fixados em formalina e embebidos em parafina foram corados para mercadores de celulas B e T. Anticorpos monoclonais para as cadeias leves de imunoglobulina kappa e lambda foram realizados para determinar a natureza poli ou monoclonal dos tumores linfoides. Baseados no diagnostico histopatologico 11 casos orbitarios foram inflamacao cronica idiopatica (20 por cento), 22 casos como classificados corno (40 por cento) e 22 casos como linfoma (40 por cento). Do total dos 22 linfomas orbitarios, 14 expressaram apenas cadeia leve tipo lambda (36,4 por cento). As hiperplasias linfoides foram positivas ou negativas para ambas cadeias leves. A inflamacao cronica idiopatica da orbita foi positiva para mercadores B e T com no minimo 50 por cento de celulas T. As hiperplasias linfoides mostraram positividade de ate 30 por cento de celulas T enquanto os predominantemente compostos por celulas B (90 por cento). Dezessete dos 35 casos da conjuntiva representaram linfomas do tipo MALT enquanto I B foram diagnosticados como hiperplasia linfoide conjuntival. As caracteristicas histopatologicas para diferenciar linfoma MALT de hiperplasia linfoide conjuntival incluiram numero de corpusculo de Dutcher e atividade mitotica. A infiltracao epitelial foi encontrada em ambos linfoma do tipo MALT e hiperplasia linfoide. Todos os 17 linfomas do tipo MALT demonstraram no minimo 80 por cento de celulas...(au)
