<br>Les infections nosocomiales en unité de soins intensifs néonatals (USIN) sont responsables d’une forte mortalité infantile. Les éviers sont des réservoirs potentiels d’agents pathogènes opportunistes associés aux réseaux d’eau hospitaliers, mais leur rôle dans la transmission des infections nosocomiales en USIN demeure mal compris car difficile à estimer. Ce projet vise à caractériser la dynamique de trois bactéries pathogènes opportunistes dans les éviers d'hôpitaux afin de mieux cibler, comprendre et réduire les facteurs qui les favorisent. Cette thèse se concentre sur l'étude de Pseudomonas aeruginosa, Serratia marcescens, et Stenotrophomonas maltophilia, bactéries à Gram-négatif fréquemment responsables d'infections nosocomiales.</br><br> Après une introduction générale, le deuxième chapitre présente le développement d'une méthode de typage moléculaire à haut débit basée sur de courtes séquences, pour l'analyse d’espèces bactériennes cultivables ou non. Cette méthode est essentielle pour l'étude d’un grand nombre d’échantillons, tout en garantissant une grande spécificité et fiabilité.</br><br> Le troisième chapitre porte sur l’investigation épidémiologique de plusieurs vagues de colonisations et infections à S. marcescens dans une USIN, en évaluant le rôle de l’évier dans ces éclosions. Les résultats révèlent que, outre les transmissions horizontales, les drains d'évier jouent un rôle important dans la transmission de cette espèce bactérienne aux nouveau-nés. Cinq méthodes de désinfection de drain ont été testées, parmi lesquelles seules les températures élevées (≥90°C dans les drains) semblent efficaces pour réduire la présence des bactéries totales et pathogènes opportunistes.</br><br> Enfin, le quatrième chapitre rapporte les principales caractéristiques de l’écologie des trois espèces suivies, et leur dynamique spatio-temporelle dans les éviers de deux USIN, en prenant en compte divers facteurs biotiques et abiotiques. Plus de la moitié des éviers se sont révélés positifs à un moment donné et à au moins une des trois bactéries suivies. Bien qu’une faible diversité génotypique ait été observée au sein de chaque évier, une forte hétérogénéité génotypique a été observée entre les éviers d’une USIN. Ces résultats soulignent le rôle des éviers en tant que réservoirs importants d’agents pathogènes opportunistes, tout en mettant en évidence des schémas de distribution distincts pour P. aeruginosa, S. marcescens et S. maltophilia. De plus, cet axe a permis d'identifier une bactérie antagoniste aux trois bactéries pathogènes opportunistes, appartenant à l'espèce Delftia tsuruhatensis. En plus de guider une révision de certaines normes de la Régie du bâtiment du Québec, les résultats de cette étude serviront à lutter plus efficacement contre les infections nosocomiales et à orienter de futures investigations scientifiques.</br><br>Nosocomial infections in neonatal intensive care units (NICUs) are responsible for high infant mortality every year. Sinks are potential reservoirs of opportunistic pathogens, but their role in the transmission of hospital-acquired infections (HAI) in NICUs remains poorly understood because it is difficult to estimate. This project aims to characterize the dynamics of three opportunistic pathogens in NICU sinks, in order to better target, understand and reduce factors favorable to opportunistic pathogens. This thesis focuses on the study of Pseudomonas aeruginosa, Serratia marcescens, and Stenotrophomonas maltophilia, Gram-negative opportunistic pathogens frequently responsible for nosocomial infections.</br><br> Following the general introduction, the second chapter presents the development of a high-throughput short sequence typing method, for the analysis of cultivable and non-cultivable bacterial species. This method is essential for the study of large numbers of samples, while guaranteeing high specificity and reliability.</br><br> The third chapter focuses on the epidemiological investigation of several clusters of S. marcescens colonizations or infections in a NICU, assessing the role of the sink in these outbreaks. The results reveal that, in addition to horizontal transmissions, sink drains play an important role in the transmission of this species to newborns. Five drain disinfection methods were tested, of which only high temperatures (≥90°C in drains) appear to be effective in reducing the presence of bacteria and opportunistic pathogens.</br><br> Finally, the fourth chapter reports on the main ecology features of the three species under study, and their spatio-temporal dynamics in sinks from two NICUs, taking into account various biotic and abiotic factors. More than half of the sinks were found to be positive for one or more opportunistic pathogens at some sampling point. Although low genotypic diversity was observed within each sink, high genotypic heterogeneity was observed between sinks in each NICU. These results underline the role of sinks as important reservoirs of opportunistic pathogens, while highlighting distinct distribution patterns for P. aeruginosa, S. marcescens, and S. maltophilia. In addition, this study identified a bacterium antagonistic to all three opportunistic pathogens, belonging to the Delftia tsuruhatensis species. In addition to guiding a revision of certain standards of the Régie du bâtiment du Québec, the results of this study will be used to fight against HAI more effectively, and to guide future scientific investigations.</br>
